Pax-8在腹膜恶性间皮瘤及高分化乳头状间皮瘤中异常表达的免疫组织化学分析

摘　　要：目的观察配对盒基因8抗原(paired box gene 8,Pax-8)在腹膜恶性间皮瘤(malignant peritonealmesothelioma,MPM)、高分化乳头状间皮瘤(well-differentiated papillary mesothelioma,WDPM)中的表达,探讨Pax-8在MPM、WDPM与卵巢浆液性肿瘤(ovarian serous tumors,OST)鉴别中的诊断价值。方法收集MPM病例32例,WDPM 12例,良性多囊性间皮瘤6,腺瘤样瘤15例,反应性间皮12例,通过免疫组织化学方法检测这些病变中Pax-8的表达,并分析临床病理学特点。结果5例MPM(15.6%,5/32)表达Pax-8,3例弥漫性中-强阳性,2例局灶性弱-中度阳性;7例WDPM(58.3%,7/12)表达Pax-8,5例弥漫性中-强阳性,2例局灶性弱-中度阳性;1例良性多囊性间皮瘤(16.7%,1/6)Pax-8表达呈局灶性弱-中度阳性;15例腺瘤样瘤均不表达Pax-8;6例反应性间皮(50%,6/12)表达Pax-8,3例弥漫性中-强阳性,3例局灶性弱-中度阳性。结论Pax-8对MPM、WDPM与OST鉴别诊断的价值有限,尤其对于WDPM与交界性或低级别浆液性肿瘤的鉴别不可靠,需要多个标记联合使用,以降低误诊的风险。Objective To explore the expression of paired box gene 8(Pax-8)in malignant peritoneal mesothelioma(MPM)and well-differentiated papillary mesothelioma(WDPM)and its value in distinguishing MPM,WDPM from ovarian serous tumors(OST).Methods Immunohistochemistry staining was used to assess the expression of Pax-8 in 32 MPM,12 WDPM,6 benign multicystic mesothelioma,15 adenomatous tumor,and 12 reactive mesothelium specimens,and the clinical-pathological characteristics were analyzed Results Pax-8 was expressed in 5 cases(15.6%,5/32)of MPM,in which 3 showed diffusely moderate to strong positive staining and 2 focally weak to moderate positive staining.Pax-8 was expressed in 7 cases(58.3%,7/12)of WDPM,with 5 showing diffusely moderate to strong positive staining and 2 focally weak to moderate positive staining.One case(16.7%,1/6)of benign multicystic mesothelioma was focally weak to moderate positive for Pax-8.All 15 adenomatous tumors were Pax-8 negative.Six cases(50%,6/12)of reactive mesothelium expressed Pax-8,with 3 showing diffusely moderate to strong positive staining and 3 focally weak to moderate positive staining.Conclusion The value of Pax-8 expression in distinguishing MPM,WDPM from OST is limited,especially in distinguishing WDPM from borderline or low-grade serous tumors.Multiple markers need to be applied in combination to reduce the risk of misdiagnosis.
