摘 要:目的探讨低剂量双源CT颅脑灌注成像(CTP)在超急性期脑梗死中的诊断准确性。方法选择2018年2月~2021年2月收治的超急性期脑梗死患者100例进行研究,纳入对象有完整临床资料,自愿配合研究且签署知情同意书,按照随机数字表法分为观察组与对照组,每组50例。所有患者均在发病6 h内完成100 kV管电压CTP检查与低剂量70 kV管电压CTP检查,其中100 kV管电压CTP为对照组,而70 kV检查为观察组。比较两组脑血流量(CBF)、脑血容量(CBV)、平均通过时间(MTT)、达峰时间(TTP)、有效剂量(ED),以及诊断准确性。结果两组患者在脑血流量、脑血容量、平均通过时间、达峰时间上对比,差异无统计学意义(P>0.05);观察组诊断有效剂量明显高于对照组(P<0.05);观察组诊断超急性期脑梗死阳性率为94.00%,对照组则为70.00%,观察组诊断超急性期脑梗死阳性率明显高于对照组(P<0.05)。结论低剂量双源CTP诊断急性脑梗死患者,与常规脑血流量、脑血容量、平均通过时间、达峰时间无明显差异,但低剂量双源CTP检查能提高有效剂量,促使诊断准确性明显提高。Objective To investigate the diagnostic accuracy of low-dose dual source CT brain perfusion imaging(CTP)in hyperacute cerebral infarction.Methods 100 patients with hyperacute cerebral infarction in our hospital from February 2018 to February 2021 were selected for the study.The subjects had complete clinical data,voluntarily cooperated with the study and signed informed consent.They were randomly divided into observation group and control group,with 50 cases in each group.All patients completed 100 kV and 70 kV tube voltage CTP examination within 6 hours of onset.CT scan was the control group,while CTP examination was the observation group.Cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time(MTT),time to peak(TTP),effective dose(ED),and diagnostic accuracy were compared between the two groups.Results There was no significant difference in cerebral blood flow,cerebral blood volume,mean transit time and peak time between the two groups(P>0.05);The effective dose of the observation group was significantly higher than that of the control group(P<0.05);The positive rate of hyperacute cerebral infarction was 94.00%in the observation group and 70.00%in the control group.The positive rate of hyperacute cerebral infarction in the observation group was significantly higher than that in the control group(P<0.05).Conclusions Low dose dual source CTP in the diagnosis of patients with acute cerebral infarction,compared with conventional dual source CT scan,there is no significant difference in cerebral blood flow,cerebral blood volume,average transit time,peak time between the two,but low dose dual source CTP can improve the effective dose,promote the accuracy of diagnosis,it is worthy of clinical application.
