下颌第二磨牙牙体缺损的两种修复方式比较

摘　　要：目的:评价下颌第二磨牙牙体缺损IPS e.max press铸瓷高嵌体和纯钛烤瓷全冠修复效果。方法:选择下颌第二磨牙远中邻牙合面Ⅱ类洞患牙51颗,分为2组:A组25颗,采用IPS e.max press铸瓷高嵌体修复牙体缺损;B组26颗,采用纯钛烤瓷全冠修复牙体缺损。随访3 a,观察两组修复后的临床效果;采用改良美国公共卫生机构评价标准进行临床效果评价,自制患者满意度调查问卷对修复体美观性、咀嚼效率、治疗效果进行评价。结果:A组2例在修复1 a后出现修复体脱落,1例在修复2 a后远中龈壁出现继发龋,修复成功率为88%;B组3 a修复成功率为100%。两组患者修复后修复体美观性、咀嚼效率和治疗效果比较,差异无统计学意义(P均>0.05)。结论:下颌第二磨牙远中邻牙合面Ⅱ类洞采用全冠修复,临床操作简便、效果确切。Aim:To evaluate the clinical effects of IPS e.max press cast ceramic onlay and pure titanium porcelain crown in the restoration of mandibular second molar defect.Methods:A total of 51 mandibular second molars with classⅡdefect were selected and allocated into two groups,group A(25 teeth)was treated with IPS e.max press cast ceramic onlay and group B(26 teeth)was treated with pure titanium porcelain crown.After a follow-up of 3 years,the aesthetics,masticatory efficiency and treatment effect of the prosthesis were evaluated by the modified American public health agency evaluation standard.Results:In group A,2 cases had prosthetic detachment after 1 year of restoration,1 case had space in distal gingival wall 2 years after restoration,the success rate was 88%,and the success rate of group B was 100%.There was no significant difference between the two groups in aesthetics,masticatory efficiency or therapeutic effect(P>0.05).Conclusion:It is simple and effective to repair the defect of mandibular second molar with full crown.
