摘 要:目的探析血清淀粉酶(S-Amy)、脂肪酶(Lipase)、C反应蛋白(CRP)在急性胰腺炎(AP)诊断中的应用与价值,为后续临床应用奠定基础。方法回顾性选取惠州市第三人民医院2019年10月至2020年10月收治的AP患者90例为研究对象,按照其严重程度分为轻型组(MAP组,30例)与重型组(SAP组,60例),同时选取50例非急性胰腺炎性急腹症(NAA组)患者,以及50例健康体检者作为对照组。对比分析患者组间S-Amy、Lipase、CRP等生化指标的差异,同时通过分析三项指标单项及联合应用诊断AP的灵敏度、特异度与符合率鉴定三种指标的临床诊断价值。结果比较三组患者S-Amy、Lipase、CRP水平,其中SAP患者和MAP明显高于NAA组和对照组,差异有统计学意义(P<0.05),NAA组的检查结果高于对照组,差异有统计学意义(P<0.05)。MAP组与SAP组Lipase与S-Amy指标比较,差异无统计学意义(P>0.05),MAP组的CRP显著高于SAP组,差异有统计学意义(P<0.05);单项检测结果其符合率均低于65.00%,两两联合检测时准确率均提高至75.00%以上,三者联合检测符合率高达88.89%。结论在诊断AP时进行S-Amy、Lipase、CRP检测,尤其是三者联合检测可以对患者早期病情进行有效的预判,从而为合理的治疗方案奠定基础,值得临床广泛推广使用。Objective To investigate the application and value of serum amylase,lipase,C-reactive protein(CRP)in the diagnosis of acute pancreatitis(AP),so as to lay a foundation for subsequent clinical application.Methods A total of 90 patients with AP admitted to and treated in the Third People's Hospital of Huizhou City from October 2019 to October 2020 were retrospectively selected as the research objects.And they were divided into the mild AP group(the MAP group,n=30)and the severe AP group(the SAP group,n=60)according to their severity.At the same time,patients with AP caused by non-acute abdomen(the NAA group,n=50)were selected,and healthy people were selected(the control group,n=50).The differences of S-Amy,lipase,CRP and other biochemical indices in several groups of patients were compared and analyzed.At the same time,the clinical diagnostic values of the three indices were identified by analyzing the sensitivity,specificity and coincidence rate of single and combined application of the three indices in the diagnosis of AP.Results The levels of S-Amy,CRP and lipase in several groups of patients were compared,the SAP and MAP groups of patients were significantly higher than those in the NAA group and the control group,with statistically significant differences(P<0.05).And results of the NAA group were significantly higher than those of the control group,with statistically significant differences(P<0.05).The MAP group and the SAP group were compared,and there were no statistically significant differences in lipase and S-Amy(P>0.05).Meanwhile,CRP results showed that the MAP group was significantly higher than the SAP group,with statistically significant difference(P<0.05).The coincidence rate of single detection results was lower than 65.00%,the accuracy rate of pairwise combined detection was increased to over 75.00%,and the coincidence rate of the three combined detection could reach 88.89%.Conclusion The detection of S-Amy,lipase and CRP in the diagnosis of AP,especially the combined detection,can effectively predi
