摘  要:目的探讨胃癌患者的血清外泌体肿瘤蛋白53靶基因1(TP53TG1)水平及临床意义。方法基于Ualcan在线分析415例原发性胃癌组织和34例正常组织的TP53TG1水平,采用Kaplan-Meier Plotter在线分析组织TP53TG1水平与胃癌预后的关系。采用实时定量PCR(qPCR)检测103例胃癌患者和79例健康体检者的血清外泌体TP53TG1水平,分析外泌体TP53TG1水平与胃癌临床病理特征和预后的关系。结果原发性胃癌组织的TP53TG1水平[中位数(25%~75%四分位数范围)]为20.765(13.931~29.195),低于正常组织的27.706(23.521~33.300),差异有统计学意义(P<0.05)。TP53TG1水平与胃癌的总生存期(OS)、首次进展后生存期(FP)和疾病进展后生存期(PPS)均有关,其中低水平者的中位OS、FP和PPS分别为22.2、13.4和8.1个月,短于高水平者的33.6、24.5和9.7个月,差异有统计学意义(P<0.05)。胃癌患者的外泌体TP53TG1水平为0.443(0.291~0.609),低于健康体检者的1.575(1.245~2.203),差异有统计学意义(P<0.05)。外泌体TP53TG1水平与肿瘤直径、Lauren分型、TNM分期和N分期有关(P<0.05),而与年龄、性别、T分期和M分期无关(P>0.05)。外泌体TP53TG1高水平组的中位OS未达,优于低水平组的52.0(95%CI:46.671~57.329)个月,差异有统计学意义(P<0.05),进一步行多因素分析发现外泌体TP53TG1水平是胃癌患者OS的危险因素(HR=2.512,95%CI:1.539~4.267,P=0.014)。结论外泌体TP53TG1水平降低与胃癌进展相关,其异常表达与胃癌患者的预后不良密切相关,外泌体TP53TG1有望成为胃癌诊断和预后评估的潜在指标。Objective To investigate the level and clinical significance of serum exosomal tumor protein 53 target gene 1(TP53TG1)in patients with gastric cancer.Methods TP53TG1 levels in 415 cases of primary gastric cancer tissues and 34 cases of normal tissues were analyzed online based on Ualcan.Relationship between the tissue TP53TG1 level and prognosis of gastric cancer was analyzed online by Kaplan-Meier Plotter.Levels of serum exosomal TP53TG1 in 103 patients with gastric cancer and 79 healthy subjects were detected by real-time quantitative PCR(qPCR).The relationship of serum exosomal TP53TG1 with clinicopathological features and prognosis of gastric cancer was analyzed.Results The TP53TG1 level[median(25%-75%quartile range)]in primary gastric cancer tissues was 20.765(13.931-29.195),lower than 27.706(23.521-33.300)in normal tissues(P<0.05).The TP53TG1 level was related to the overall survival(OS),first progression(FP)and post-progression survival(PPS)of gastric cancer.The median OS,FP and PPS of patients with low-level TP53TG1 were 22.2,13.4 and 8.1 months,shorter than 33.6,24.5 and 9.7 months of patients with high-level TP53TG1(P<0.05).Exosomal level of TP53TG1 in patients with gastric cancer was 0.443(0.291-0.609),lower than 1.575(1.245-2.203)in healthy subjects(P<0.05).Hierarchical analysis showed that the TP53TG1 level was related to tumor diameter,Lauren classification,TNM stage and N stage(P<0.05),but not to age,sex,T stage and M stage(P>0.05).The median OS in the high-level of exosomal TP53TG1 group was unreached,superior to 52.0(95%CI:46.671-57.329)months in the low-level of exosomal TP53TG1 group(P<0.05).Further multivariate analysis found that the level of exosomal TP53TG1 was a risk factor for OS(HR=2.512,95%CI:1.539-4.267,P=0.014).Conclusion Decreased exosomal TP53TG1 is related to the progression of gastric cancer,and its abnormal expression is closely related to the poor prognosis of gastric cancer patients.Exosomal TP53TG1 is expected to be a potential index for the diagnosis and prognosis of gastric
