临床实习生临终关怀“认知、态度”现状调查及影响因素分析

摘　　要：目的调查临床实习生对临终关怀相关知识的认知现状,并分析相关指标偏差的影响因素。方法于2019年3月采用自制问卷对本院临床实习的229名学生进行调查,了解其对临终关怀相关知识的掌握情况、对待死亡和安乐死的态度、从事临终关怀相关工作的意愿以及临终关怀的发展前景。结果临床实习生对临终关怀的相关知识掌握普遍欠缺,仅有3.9%的学生对临终关怀的概念非常了解,性别、有无宗教信仰和有否接触过临终患者对临终关怀概念的了解比较,差异有统计学意义(P<0.05);不同的性别、专业以及受教育程度的高低对止痛剂及吗啡不良反应的了解比较,差异有统计学意义(P<0.05);公开谈论死亡话题的学生只有6.1%,文化程度的高低对安乐死的了解比较,差异有统计学意义(P<0.05)。结论临床实习生对临终关怀相关知识普遍缺乏,亟需构建临终关怀教育的核心课程,加强对临床实习生的规范化培训。Objective To investigate the status of cognition and attitude on hospice care among clinical interns and and analyze the influence factors of correlation index deviation.Methods In March 2019,a survey using self-designed questionnaire was conducted among 229 students those practice in our hospital,to understand their grasp of the hospice care knowledge,attitudes towards death and euthanasia,their willing engaged in the work of hospice care,as well as the prospects of the development of hospice care.Results Clinical interns generally lack the knowledge of hospice care,only 3.9%of the students understand the concept of hospice care very well.There were statistically significant differences in understanding the concept of end-of-life care among patients of different gender,with or without religious belief,and whether ever contact with end-of-life care(P<0.05).There were statistically significant differences in the understanding of analgesics and morphine adverse reactions among students of different gender,majors and education levels(P<0.05).Only 6.1%of the students talked about death in public,and the difference of their education level in understanding euthanasia was statistically significant(P<0.05).Conclusions Clinical interns generally lack knowledge about hospice care,so it is urgent to construct core courses of hospice care education and strengthen the standardized training of clinical interns.
