《寓意草》治痢法对临证久泻辨治启示

摘　　要：[目的]总结《寓意草》中体现的治痢思想,探索其对久泻临证辨治的指导意义,并总结纪云西教授辨治久泄的经验。[方法]通过研读《寓意草》辨治痢疾的8则医案,探析喻氏的治痢法度,并总结纪云西教授借鉴喻氏治痢方法辨治久泻的经验。[结果]《寓意草》所载治痢医案体现了喻氏安胃为要、顾护胃气,和润缓图、慎用急药,从表解痢、防陷阴分,通补结合、温通为主的治痢思想。纪云西教授借鉴喻氏治痢之法,在临证辨治久泻时注重开表宣肺、和胃化湿,疏肝理气、温中健脾,益肾填精、甘缓滋补。[结论]喻氏治痢强调扶正透表,通补结合。纪云西教授将喻氏治痢法引申于临床,取得良好疗效,可供临床借鉴。[Objective]To summarize the theory of dealing with dysentery in"Yuyicao"and explore the guiding significance in treating long-term dysentery.At the same time,summarize Professor JI Yunxi’s experience in treating chronic diarrhea.[Methods]Through analyzing YU Chang’s 8 clinical cases of treating dysentery,this paper explored YU Chang’s principle in treating dysentery and summarized Professor JI Yunxi’s experience in differentiating and treating dysentery with YU Chang’s methods.[Results]The cases in"Yuyicao"revealed that YU Chang was good at taking soothing the stomach firstly,treating dysentery through protecting the stomach Qi,using sweet-mild drugs,carefully using emergency medicine,treating dysentery from the exterior,preventing Yin from depression,combining purgation and compensation and taking warming purgation as the main methods in treating dysentery.Professor JI Yunxi learns from YU and uses the methods of relieving exterior syndrome and dispersing the lung,harmonizing the stomach and cleaning the dampness,soothing the liver and regulating Qi,warming the stomach and invigorating the spleen,nourishing the kidney and filling essence,and using sweet-mild drugs.[Conclusion]YU Chang emphasized strengthening the body resistance,treating dysentery from the exterior,and combining purgation and compensation.Professor JI Yunxi uses YU Chang’s theory of dysentery treatment in clinical practice and gets good curative effect,which can be used for reference in clinical practice.
