摘 要:目的探讨微管相关蛋白轻链3-Ⅱ(LC3Ⅱ)与白细胞分化抗原44(CD44)在结直肠癌及其浸润性成分中的表达,分析两者在结直肠癌发生发展及转移中的作用。方法从天津医科大学第二医院病理科2011年至2014年结直肠癌手术切除标本中选取64例结直肠癌、20例结直肠腺瘤和20例癌旁正常组织(距癌组织边缘>10 cm)作为研究对象,利用免疫组织化学染色检测20例癌旁组织、20例腺瘤及64例结直肠癌组织中CD44蛋白及LC3Ⅱ蛋白的表达,分析两者与结直肠癌临床病理学特征的关系、在结直肠浸润性成分中表达的差异及两者的相关性。通过Fishier确切概率法分析LC3Ⅱ与CD44蛋白的表达与癌旁组织、腺瘤及结直肠癌不同病理参数之间的关系。结果 (1)LC3Ⅱ在结直肠癌中的表达高于癌旁组织及腺瘤中(χ^(2)=66.495,P<0.01),但其在癌旁组织及腺瘤中的表达差异无统计学意义(χ^(2)=3.198,P>0.05);CD44在癌旁组织、腺瘤及结直肠癌的表达逐渐增高(χ^(2)=47.782,P<0.01)。(2)LC3Ⅱ和CD44在结直肠癌中的表达与肿瘤分化程度、淋巴结转移、伴癌结节及临床分期有关(依次为LC3Ⅱ:χ^(2)=8.313、8.459、9.356、10.011,P<0.05;CD44:χ^(2)=9.182、9.430、9.560、13.513,P<0.05),LC3Ⅱ的表达与肿瘤浸润深度无关(χ^(2)=4.523,P>0.05),CD44的表达与浸润深度有关(χ^(2)=8.322,P<0.05)。(3)肿瘤浸润性成分中LC3Ⅱ和CD44的表达水平高于癌灶中心(LC3Ⅱ:χ^(2)=26.004,P<0.01;CD44:χ^(2)=26.145,P<0.01),在结直肠癌中两者的表达呈正相关(r=0.492,P<0.05)。结论 LC3Ⅱ和CD44在结直肠癌发生发展中均发挥重要作用,两者协同作用共同导致结直肠癌的不良预后。Objective To investigate the expression of microtubule-associated protein light chain 3-Ⅱ(LC3Ⅱ)and cluster of differentiation 44(CD44)in colorectal cancer and its invasive components,and to analyze their roles in the development,progression and metastasis of colorectal cancer.Methods Immunohistochemical staining was used to detect the expression of CD44 and LC3Ⅱin 20 cases of paracancerous tissues,20 cases of adenomas and 64 cases of colorectal cancers.To analyze the relationship between them and clinicopathological features of colorectal cancer,the difference of expression in invasive components of colorectal cancer and their correlation.Results(1)The expression of LC3Ⅱin colorectal cancer was higher than that in paracancerous tissue and adenoma(χ^(2)=66.495,P<0.01),but there was no difference between paracancerous tissue and adenoma(χ^(2)=3.198,P>0.05).The expression of CD44 was gradually increased in paracancerous tissues,adenomas,and colorectal cancers(χ^(2)=47.782,P<0.01).(2)The expression of LC3Ⅱand CD44 in colorectal cancer was related to the degree of tumor differentiation,lymph node metastasis,tumor nodule and clinical stage(LC3Ⅱ:χ^(2)=8.313,8.459,9.356,10.011,P<0.05;CD44:χ^(2)=9.182,9.430,9.560,13.513,P<0.05).The expression of LC3Ⅱwas not related to the depth of tumor invasion(χ^(2)=4.523,P>0.05).The expression of CD44 was related to the depth of infiltration(χ^(2)=8.322,P<0.05).(3)The expression of LC3Ⅱand CD44 in tumor invasive components was higher than that in the center of tumor(LC3Ⅱ:χ^(2)=26.004,P<0.01;CD44:χ^(2)=26.145,P<0.01).The expression of them is positively correlated in colorectal cancer(r=0.492,P<0.05).Conclusion Both LC3Ⅱand CD44 play an important role in the development of colorectal cancer,and they act synergistically and lead to poor prognosis of colorectal cancer.
