摘  要:射频消融(RFA)是肝细胞癌(HCC)潜在的治愈手段之一。HCC的病理特点、肿瘤的特殊位置及操作技术等原因导致RFA治疗HCC时未能达到病理性完全消融,进而产生肿瘤残留。残留癌侵袭、转移等能力增强是影响HCC远期疗效的重要因素。HCC RFA后残留癌进展的机制涉及炎症、上皮-间充质转化、肿瘤细胞干细胞特性增强、自噬、肿瘤微环境等。本文就HCC RFA后残留癌进展的机制及治疗策略进行综述,以期为提高HCC RFA疗效提供研究思路。Radiofrequency ablation(RFA)is one of the potential treatments for hepatocellular carcinoma(HCC).Pathological characteristics,special tumor location and surgical techniques of HCC may lead to failure of pathological complete ablation during RFA therapy for HCC,resulting in tumor residues.The enhanced ability of residual tumor invasion and metastasis is an important factor affecting the long-term efficacy of HCC.The mechanisms of residual tumor progression after RFA are involved in inflammation,epithelial mesenchymal transformation,enhancement of tumor stem cell characteristics,autophagy,and tumor microenvironment and so on.In this paper,the mechanisms and treatment strategies of residual tumor progression after insufficient RFA were reviewed in order to provide innovative ideas for improving the efficacy of HCC after RFA.
