腹股沟毛囊漏斗部肿瘤1例并文献复习

摘　　要：患者女,62岁,左侧腹股沟出现一黄豆大小褐色斑疹1年余。皮损组织病理示:表皮轻度角化,棘层肥厚,上皮细胞组成的细胞团块和条索结构位于真皮浅层内,相互吻合并与表皮相连,境界清楚,呈水平开孔样生长模式,孔周为呈栅栏状排列的基底样细胞,肿瘤细胞未见明显异型性。基底层附近细胞排列紊乱伴裂隙形成,可见色素失禁,真皮浅层纤维基质和成纤维细胞增生,淋巴细胞和组织细胞炎症浸润。免疫组织化学示:Bcl-2(+),CK5/6(+),Ki-67<5%。诊断:毛囊漏斗部肿瘤。治疗:手术切除并长期随访。A 62-year-old female patient presented with a bean-sized brown lesion in her left groin for more one year.The histopathology of the lesion showed mild hyperkeratosis and hypertrophy of the spinous layer.Cell masses and cord-like structures were composed of epithelial cells located in the superficial dermis,anastomosing each other and connecting to the epidermis,with clear boundaries and a horizontal aperture growth pattern,surrounded by basal-like cells arranged in a fenestrated pattern,no obvious atypia.Disturbed cell arrangement and fissure formation near the basal layer with pigment incontinence,hyperplastic fibrous stroma and fibroblasts in the superficial dermis,inflammatory cell infiltration of lymphocytes and histiocytes also could be seen.Immunohistochemical staining showed Bcl-2(+),CK5/6(+),Ki-67<5%.Tumor of follicular infundibulum was diagnosed.Treatment surgical excision and long-term followed-up.
