摘　　要：目的探讨S100钙结合蛋白A4(S100A4)、Toll样受体7(TLR7)及细胞核因子-κBp65(NF-κBp65)在子宫内膜癌中表达及对病情进展和预后的评估作用。方法收集2015年3月—2018年3月收治的78例子宫内膜癌为子宫内膜癌组,并选取同期行手术治疗的子宫内膜不典型增生85例为对照组。比较两组S100A4、TLR7及NF-κBp65表达情况,并分析三者与子宫内膜癌病理特征的关系。随访3年,比较不同S100A4、TLR7及NF-κBp65表达子宫内膜癌患者的预后情况,并分析三者对患者预后死亡的预测价值。结果子宫内膜癌组S100A4、TLR7、NF-κBp65阳性率均高于对照组(P<0.01)。国际妇产科联盟(FIGO)分期Ⅲ~Ⅳ期、有淋巴结转移的子宫内膜癌患者S100A4、TLR7、NF-κBp65阳性率分别高于FIGO分期Ⅰ~Ⅱ期、无淋巴结转移患者(P<0.05,P<0.01)。S100A4、TLR7、NF-κBp65阳性表达患者病死率分别高于S100A4、TLR7、NF-κBp65阴性表达患者(P<0.05,P<0.01)。S100A4、TLR7、NF-κBp65及三者联合检测预测子宫内膜癌患者预后死亡的曲线下面积分别为0.958、0.754、0.832、0.966;三者联合检测的敏感度、特异度高于S100A4、TLR7、NF-κBp65单一检测(P<0.05)。结论S100A4、TLR7及NF-κBp65在子宫内膜癌组织中高表达,与病情进展和预后相关,三者联合检测可评估患者预后。Objective To explore expressions of S100 calcium-binding protein A4(S100A4),Toll like receptor 7(TLR7)and nuclear factor-κBp65(NF-κBp65)in endometrial cancer and their effects in evaluation of progression and prognoses of the disease.Methods A total of 78 patients with endometrial cancer admitted between March 2015 and March 2018 were collected as endometrial cancer group,while other 85 patients with endometrial atypical hyperplasia(EAH)undergoing surgical treatment at the same period were selected as control group.Expressions of S100A4,TLR7 and NF-κBp65 were compared,and correlations between the three indexes with pathological characteristics of endometrial cancer were analyzed in two groups.The patients were followed up for 3 years,and prognosis conditions of endometrial cancer patients with different expressions of S100A4,TLR7 and NF-κBp65 were compared,and the values of the three indexes in prognosis of patients'death were analyzed.Results Positive rates of S100A4,TLR7 and NF-κBp65 in endometrial cancer group were significantly higher than those in control group(P<0.01).Positive rates of S100A4,TLR7 and NF-κBp65 in endometrial cancer patients with stages III to IV by the International Federation of Obstetrics and Gynecology(FIGO)and having lymph node metastasis were significantly higher than those with stages I to II by FIGO and no lymph node metastasis(P<0.05,P<0.01).The mortality rate of patients with positive expressions of S100A4,TLR7 and NF-κBp65 was significantly higher than that of patients with negative expressions of S100A4,TLR7 and NF-κBp65(P<0.05,P<0.01).The areas under the receiver operating curve(ROC)(AUC)of S100A4,TLR7,NF-κBp65 and the combined detection of the three indexes in prediction of prognostic death of endometrial cancer patients were 0.958,0.754,0.832 and 0.966 respectively.The sensitivity and specificity of the combined detection of the three indexes were significantly higher than those by S100A4,TLR7 and NF-κBp65 examinations alone(P<0.05).Conclusion S100A4,TLR7 and NF-κBp
