聚乙二醇化重组人粒细胞集落刺激因子在外周血造血干细胞移植中的应用观察

摘　　要：目的探讨聚乙二醇化重组人粒细胞集落刺激因子在外周血造血干细胞移植术后对刺激粒细胞生长的有效性、安全性及经济效应。方法回顾性分析94例血液肿瘤患者行外周血造血干细胞移植术,其中PEG-rhG-CSF组患者50例,在自体干细胞移植术后第1天或异体干细胞移植术后第5天予PEG-rhG-CSF 6 mg皮下注射;rhG-CSF组患者44例,在自体干细胞移植术后第1天或异体干细胞移植术后第5天予rhG-CSF 300μg Qd皮下注射,至中性粒细胞绝对计数>1.5×10^(9)/L停药或减量。结果PEG-rhG-CSF组和rhG-CSF组在外周血干细胞移植中年龄、粒细胞植活时间差异无统计学意义(P≥0.05),一般资料中性别、诊断、化疗方案、移植类型差异存在统计学意义(P<0.05),粒细胞缺乏持续时间、粒细胞缺乏伴发热、粒细胞生长期疼痛分度及用药费用间差异均存在统计学意义(P<0.05)。结论PEG-rhG-CSF和rhG-CSF对外周血造血干细胞移植后粒细胞植活时间差异无统计学意义,PEG-rhG-CSF组患者用药价格高于rhG-CSF组,但PEG-rhG-CSF一个化疗周期仅皮下注射一次,骨关节肌肉疼痛发生率及疼痛程度比较也相对更低,能减轻患者痛苦。Objective To investigate the efficacy,safety and economic effect of polyethylene glycolated recombinant human granulocyte colony stimulating factor in stimulating granulocyte growth after peripheral blood hematopoietic stem cell transplantation.Methods A retrospective analysis was performed on 94 patients with hematologic tumor who underwent peripheral blood hematopoietic stem cell transplantation.These patients were divided into the PEG-rhG-CSF group(50 patients)and rhG-CSF group(44 patients).The PEG-rhG-CSF group received 6mg subcutaneous injection of PEG-rhG-CSF on the 1st day after autologous stem cell transplantation or the 5th day after allogeneic stem cell transplantation,while the RhG-CSF group received a subcutaneous injection of rhG-CSF 300μg Qd on the first day after autologous stem cell transplantation or the fifth day after allogeneic stem cell transplantation,and stopped or reduced the dose until the absolute neutrophils count was greater than 1.5×10^(9)/L.Results There were no significant differences in the age of peripheral blood stem cell transplantation and granulocyte survival time between 2 groups(P≥0.05).There were statistical differences in gender,diagnosis,chemotherapy regimen and transplantation type in general data(P<0.05).There were statistical differences in the duration of granulocytosis,granulocytosis with fever,grade of pain during granulocytosis growth and drug cost(P<0.05).Conclusion PEG-rhG-CSF and rhG-CSF show no significant difference in granulocytes survival time after peripheral blood hematopoietic stem cell transplantation.The drug price of the PEG-rhG-CSF group is higher than that of the rhG-CSF group.However,PEG-rhG-CSF is only injected subcutaneously once per chemotherapy cycle,and the incidence and severity of bone,joint and muscle pain are lower than that of the rhG-CSF group,which could relieve the pain of the patients.
