摘　　要：目的探讨WT1基因在成人初诊急性髓系白血病(AML)不同预后分层中的表达特点及临床意义。方法采用RT-PCR检测80例成人初诊AML患者WT1基因表达水平,按预后危险分层标准将其分为低、中、高危三组,比较各组WT1基因表达特点,观察有无临床意义。结果初诊AML治疗前WT1表达水平在各亚型中差异无统计学意义(P>0.05);AML低、中危两组WT1表达水平比较差异无统计学意义(P>0.05),中、高危两组差异有统计学意义(P<0.05),低、高危两组比较差异有统计学意义(P<0.05);初诊时WT1高表达与低表达组的CR率比较差异无统计学意义(P>0.05)。结论 WT1基因有可能成为协助判断AML患者危险分层的指标之一,WT1基因高表达可能是AML预后不良的因素;初诊时WT1基因表达水平对CR率无影响。Objective To investigate the expression characteristics and clinical significance of WT1 gene in different prognostic stratification of adult newly diagnosed acute myeloid leukemia. Methods The expression level of WT1 gene in 80 newly diagnosed adult patients with acute myeloid leukemia(AML) was detected by RT-PCR. The patients were divided into three groups(low, medium and high risk) according to the prognostic risk stratification criteria. The expression characteristics of WT1 gene in each group were compared, and observed whether there was clinical significance. Results There were no significant differences in the expression level of WT1 among the subtypes before treatment of newly diagnosed AML(P>0.05). There was no significant difference in WT1 expression level between the low and medium risk AML groups(P>0.05), while there was significant difference between the medium and high risk AML groups(P<0.05), and there was significant difference between the low and high risk AML groups(P<0.05).There was no significant difference in CR rate between the high WT1 expression group and low WT1 expression group at the initial diagnosis(P>0.05). Conclusion WT1 gene may be one of the indicators to help judge the risk stratification of AML patients, and the high expression of WT1 gene may be a factor of poor prognosis of AML. The expression level of WT1 gene has no effect on CR rate at the initial diagnosis.
