补阳还五汤配合针灸治疗脑卒中偏瘫患者的效果及其对康复期肢体恢复的影响

摘　　要：目的探讨针灸联合补阳还五汤在治疗脑卒中偏瘫患者中的应用效果。方法选取2020年1月至12月聊城市第二人民医院收治的96例脑卒中偏瘫患者作为研究对象,采用随机数字表法分为对照组(48例)和观察组(48例)。对照组采用西医常规治疗,观察组在此基础上采用补阳还五汤配合针灸治疗,两组均连续治疗4周。比较两组的临床疗效、肢体功能、日常生活能力、生命质量、炎性指标水平[基质金属蛋白酶9(MMP-9)和超敏C反应蛋白(hs-CRP)]及不良反应总发生率。结果观察组的临床总有效率高于对照组,差异有统计学意义(P<0.05);两组治疗后的Barthel指数评定量表(MBI)、Fugl-Meyer运动功能评分法(FMA)、脑卒中专门化生命质量量表(SS-QOL)评分高于治疗前,MMP-9、hs-CRP水平低于治疗前,差异有统计学意义(P<0.05);观察组治疗后的MBI、FMA、SS-QOL评分高于对照组,MMP-9、hs-CRP水平低于对照组,差异有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论在脑卒中偏瘫患者中联合应用补阳还五汤和针灸治疗安全、有效,能改善患者肢体功能、日常生活能力及生命质量,减轻炎症反应。Objective To analyze the effect of combined treatment of acupuncture and Buyang Huanwu Decoction in stroke patients with hemiplegia.Methods A total of 96 stroke patients with hemiplegia admitted to Liaocheng Second People′s Hospital from January 2020 to December 2020 were selected as the research objects,and they were divided into the control group(48 cases)and the observation group(48 cases)according to the random number table method.The control group was treated with conventional Western medicine,and the observation group was treated with Buyang Huanwu Decoction and acupuncture on this basis.Both groups were treated for four consecutive weeks.The clinical efficacy,limb function,ability of daily living,quality of life,inflammatory index levels(matrix metalloproteinase 9[MMP-9]and high-sensitivity C-reactive protein[hs-CRP])and adverse reactions were compared between the two groups.Results The total clinical effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the two groups were treated with modified Barthe index(MBI),Fugl-Meyer motor assessment(FMA),stroke special quality of life scale(SS-QOL)score was higher than those before treatment,and the levels of MMP-9 and hs-CRP were lower than those before treatment,the differences were statistically significant(P<0.05).After treatment,the scores of MBI,FMA,SS-QOL in the observation group were higher than those of the control group,and the levels of MMP-9 and hs-CRP were lower than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The combined use of Buyang Huanwu Decoction and acupuncture treatment in patients with stroke and hemiplegia is safe and effective.It can improve the limb function,ability of daily living and quality of life,and reduce inflammation.
