摘  要:目的:探讨广泛性焦虑障碍(GAD)在脑灰质体积改变的基础上是否存在功能连接(FC)异常及其与症状的相关性。方法:比较38例GAD患者(病例组)及30名性别、年龄、受教育程度相匹配的健康对照者(对照组)基于感兴趣区的全脑体素FC差异;分析种子区与全脑体素FC与汉密尔顿焦虑量表(HAMA)评分的相关性。结果:与对照组相比,病例组右侧眶部额下回脑区与右侧颞中回、右侧内侧额上回、左侧楔前叶、左侧角回及左侧额中回的FC减少,与左侧颞上回的FC增加(P<0.001,AlphaSim校正);左侧颞中回与右侧舌回FC减少,与左侧楔前叶的FC增加(P<0.001,AlphaSim校正)。右侧眶部额下回与全脑体素FC强度与HAMA评分无明显相关性(P<0.001,AlphaSim校正);左侧颞中回与右侧缘上回、右侧顶下小叶及右侧额上回的FC与HAMA评分呈正相关(P均<0.001,AlphaSim校正)。结论:GAD患者可能存在多重功能网络异常;颞中回同缘上回、顶下小叶及额上回的FC强度可能反映了GAD患者临床症状的内表型。Objective:To explore whether generalized anxiety disorder(GAD)has functional connectivity(FC)abnormalities based on changes in brain gray matter volume and its correlation with symptoms.Method:Comparison of the differences in the FC of the whole brain voxels based on the region of interest was conducted between 38 patients with GAD(case group)and 30 healthy controls(control group)matched with gender,age and education level.The correlation between the FC of the seed area and the whole brain voxels and Hamilton Anxiety Scale(HAMA)scores was analyzed.Results:Compared with the control group,the FCs between the right inferior orbital frontal gyrus and the right middle temporal gyrus,the right medial superior frontal gyrus,the left precuneus,the left angular gyrus,the left middle frontal gyrus reduced in the case group,while the FCs with the left superior temporal gyrus increased(P<0.001,AlphaSim correction).The FC between the left middle temporal gyrus and the right lingual gyrus decreased,while the FC with the left anterior wedge increased(P<0.001,AlphaSim correction).There was no significant correlation between the FC strength of the right orbital subfrontal gyrus and the global brain voxel and the HAMA score(P<0.001,AlphaSim correction).The FCs between the left middle temporal gyrus and the right superior marginal gyrus,the right inferior parietal lobule and the right superior frontal gyrus were positively correlated with the HAMA score(P<0.001,corrected by alphasim).Conclusion:GAD patients may have multiple functional network abnormalities.The intensity of FC in the superior temporal gyrus,inferior parietal lobule and superior frontal gyrus may reflect the endophenotype of clinical symptoms.
