摘　　要：目的研究肝细胞癌患者血清中Merlin蛋白的表达及其与肝癌易感性、化疗敏感性及预后的关系。方法采用酶联免疫吸附法检测HCC患者和健康者血清中Merlin蛋白的表达水平,分析肝细胞癌患者Merlin蛋白的表达水平与患者临床资料的相关性。测定患者化疗后Merlin蛋白表达水平,根据化疗效果将患者分为完全缓解(CR)、部分缓解(PR)、疾病稳定(SD)和疾病进展(PD)4组,比较各组患者Merlin蛋白表达情况。采用多因素Logistic回归分析肝细胞癌患者预后情况的影响因素。结果肝细胞癌患者血清Merlin蛋白的表达水平高于健康人(P<0.001)。肿瘤大小、有无转移、分化程度和外周血甲胎蛋白(Alpha-fetoprotein,AFP)水平与患者血清Merlin蛋白的表达水平密切相关(P<0.05)。患者化疗后血清Merlin蛋白的表达水平明显下降且化疗效果越差的患者Merlin蛋白的表达水平越高(P<0.001)。多因素Logistic回归分析结果显示HCC的肿瘤直径、分化程度、有无淋巴结转移、外周血AFP水平、血清Merlin蛋白水平与肝癌患者预后显著相关(P<0.05)。结论肝细胞癌患者血清中Merlin蛋白的表达水平显著升高,且与肝细胞癌的发生、发展及化疗效果密切相关。Objective To study the expression of serum Merlin protein in patients with hepatocellular carcinoma and its relationship with susceptibility to liver cancer,chemotherapy sensitivity and prognosis.Methods Enzyme-linked immunosorbent assay was used to detect the serum Merlin protein expression level of HCC patients and healthy people,and analyze the correlation between the Merlin protein expression level of hepatocellular carcinoma patients and the clinical data of the patients.Determine the level of Merlin protein expression in patients after chemotherapy,and divide the patients into complete remission(CR),partial remission(PR),stable disease(SD)and disease progression(PD)according to the chemotherapy effect,and compare the Merlin protein expression of patients in each group.Multivariate Logistic regression was used to analyze the factors affecting the prognosis of patients with hepatocellular carcinoma.Results The expression level of serum Merlin protein in patients with hepatocellular carcinoma was higher than that of normal tissues(P<0.001).Tumor size,metastasis,degree of differentiation,and peripheral blood alpha-fetoprotein(AFP)levels were closely related to the patient's serum Merlin protein expression levels(P<0.05).The expression level of serum Merlin protein decreased significantly after chemotherapy,and the expression level of Merlin protein was higher in patients with worse chemotherapy effect(P<0.001).The results of multivariate logistic regression analysis showed that HCC tumor diameter,degree of differentiation,lymph node metastasis,peripheral blood AFP level,and serum Merlin protein level were significantly related to the prognosis of liver cancer patients(P<0.05).Conclusion The increase of serum expression level of Merlin protein is closely related to the occurrence and development of hepatocellular carcinoma and the effect of chemotherapy.
