摘　　要：目的探讨子宫内膜癌手术后人附睾蛋白4(HE4)和糖类抗原19-9(CA19-9)水平变化与患者预后的相关性。方法收集60例子宫内膜癌患者为调查研究对象纳入观察组,患者均进行手术治疗,另选同期行健康检查的健康受试者50例为对照组。对观察组手术前后、对照组的HE4、CA19-9水平进行检测对比。对观察组患者进行随访观察,根据患者的预后效果分为预后良好组和预后不良组,对两组患者的HE4、CA19-9、无进展生存期(PFS)、总生存期(OS)进行统计对比。并对子宫内膜癌患者的HE4、CA19-9水平变化与预后效果的相关性进行分析探讨。结果观察组手术后HE4、CA19-9显著低于手术前,但均高于对照组相应指标水平(P<0.05)。预后良好组患者的HE4、CA19-9水平低于预后不良组,PFS、OS高于预后不良组(P<0.05)。经Pearson检验分析,子宫内膜癌患者HE4、CA19-9水平与预后指标PFS、OS呈负相关性(P<0.05)。结论子宫内膜癌手术后HE4、CA19-9较术前均有显著降低,且预后良好组HE4、CA19-9低于预后不良组,HE4、CA19-9与患者的预后呈现明显的相关性,可作为判断子宫内膜癌患者预后的参考指标。Objective To investigate the correlation between the changes of human epididymal protein 4(HE4)and carbohydrate antigen 19-9(CA19-9)levels and prognosis of patients with endometrial carcinoma after operation.Methods 60 patients with endometrial cancer were collected as the research objects and included in the observation group.All patients underwent surgical treatment.Another 50 healthy subjects who underwent health examinations during the same period were selected as the control group.The levels of HE4 and CA19-9 in the observation group and the control group were detected and compared.The patients in the observation group were followed up and divided into good prognosis group and poor prognosis group according to the prognosis effect.The HE4,CA19-9,PFS and OS of the 2 groups were statistically compared.The correlation between the changes of HE4,CA19-9 levels and prognosis in patients with endometrial carcinoma was analyzed.Results After operation,HE4 and CA19-9 in the observation group were significantly lower than those before operation,but higher than those in the control group(P<0.05).The levels of HE4 and CA19-9 in good prognosis group were lower than those in poor prognosis group,PFS and OS were higher than those in poor prognosis group(P<0.05).Pearson test showed that the levels of HE4 and CA19-9 were negatively correlated with PFS and OS(P<0.05).Conclusion The levels of HE4 and CA19-9 in patients with endometrial carcinoma after operation are significantly lower than those before operation,and the levels of HE4 and CA19-9 in patients with good prognosis are lower than those in patients with poor prognosis.HE4 and CA19-9 are significantly correlated with the prognosis of patients with endometrial cancer,which can be used as a reference index to judge the prognosis of patients with endometrial cancer.
