麦滋林-S配伍胃复春治疗对HP阴性慢性萎缩性胃炎患者EGF、EGFR的影响

摘　　要：目的探究麦滋林-S联合胃复春治疗对幽门螺旋杆菌(HP)阴性慢性萎缩性胃炎(CAG)患者表皮生长因子(EGF)、表皮生长因子受体(EGFR)的影响。方法选取2017年1月至2020年3月于铜川市中医院治疗的132例Hp阴性CAG患者为研究对象,根据随机数字表法分为A组(胃复春单一治疗)、B组(麦滋林-S单一治疗)和观察组(胃复春联合麦滋林-S治疗),每组44例,比较三组患者的治疗效果、胃镜下观察指标评分、胃组织病理评分及EGF、EGFR表达水平。结果观察组患者的治疗总有效率为93.2%,明显高于A组的56.8%和B组的59.1%,差异均具有统计学意义(P<0.05);治疗后,观察组患者的胃镜下观察指标评分和胃组织病理评分分别为(5.4±2.1)分、(10.9±2.6)分,明显低于A组[(7.1±1.9)分、(13.3±4.1)分]和B组[(7.3±2.0)分、(13.0±4.0)分],差异均具有统计学意义(P<0.05);治疗后,三组患者的血清EGF水平和胃黏膜EGFR阳性率较治疗前均有所降低,且观察组的血清EGF水平和胃黏膜EGFR阳性率分别为(1.5±0.2)μg/L、27.3%,明显低于A组[(2.0±0.3)μg/L、43.2%]和B组[(2.0±0.4)μg/L、40.9%],差异均有统计学意义(P<0.05)。结论麦滋林-S配伍胃复春治疗对Hp阴性CAG患者的疗效较好,可降低患者EGF、EGFR的表达水平,这可能是其发挥作用的机制之一。Objective To explore the effects of Maizilin-S combined with Weifuchun treatment on epidermal growth factor(EGF) and epidermal growth factor receptor(EGFR) in patients with Helicobacter pylori(HP)-negative chronic atrophic gastritis(CAG). Methods A total of 132 patients with Hp-negative CAG were selected as the research objects, who were treated in Tongchuan Traditional Chinese Medicine Hospital from January 2017 to March 2020. They were divided into group A(Weifuchun monotherapy), group B(Maizilin-S monotherapy), and observation group(dual therapy of Weifuchun and Maizilin-S) according to the random number table method, with 44 cases in each group. The treatment effects, the gastroscopic observation index scores, gastric pathological scores, and the expression levels of EGF and EGFR were compared between the three groups. Results The total effective rate of observation group was 93.2%, which was significantly higher than 56.8% of group A and 59.1% of group B(P<0.05). After treatment, the scores of gastroscopic observation index and gastric histopathology in the observation group were(5.4±2.1) points and(10.9±2.6) points, respectively, which were significantly lower than(7.1±1.9) points and(13.3±4.1) points in group A and(7.3±2.0) points and(13.0±4.0) points in group B, with statistically significant differences(P<0.05). After treatment, the serum EGF level and EGFR positive rate of gastric mucosa were lower in the three groups after treatment than those before treatment;the serum EGF level and gastric mucosal EGFR positive rate in the observation group were(1.5±0.2) μg/L and 27.3%, respectively, which were significantly lower than(2.0±0.3) μg/L and 43.2% in group A and(2.0±0.4) and 40.9% in group B, with statistically significant differences(P<0.05). Conclusion Maizilin-S combined with Weifuchun treatment has a better effect on Hp-negative CAG patients, and it can reduce the expression levels of EGF and EGFR, which may be one of its mechanisms of action.
