摘　　要：目的探讨经直肠超声弹性成像辅助下前列腺穿刺活检用于前列腺癌诊断的价值。方法回顾性分析2018年1月至2020年12月期间肇庆市第一人民医院诊治的60例疑似为前列腺癌患者的临床资料,分别经直肠常规超声及直肠超声弹性成像检查,并在两者引导下进行前列腺穿刺活检,以病理诊断作为金标准,比较常规超声和超声弹性成像对前列腺癌的诊断效能以及两者引导下的穿刺阳性率。结果超声弹性成像诊断前列腺癌的灵敏度、特异度、准确性分别为91.67%、88.89%、90.00%,明显高于常规超声的62.50%、63.64%、63.33%,差异均有统计学意义(P<0.05);超声弹性成像穿刺点阳性率为52.17%,明显高于常规超声的17.14%,差异有统计学意义(P<0.05)。结论经直肠超声弹性成像辅助下前列腺穿刺活检对诊断前列腺癌具有重要价值,与常规超声下比较,其能够提高前列腺癌检出率。Objective To analyze the value of transrectal ultrasound elastography in the diagnosis of prostate cancer and the application of prostate biopsy auxiliary guidance.Methods The clinical data of 60 patients of suspected prostate cancer treated in Zhaoqing First People's Hospital from January 2018 to December 2020 were respectively analyzed.These patients were examined by conventional transrectal ultrasound and transrectal ultrasound elastography respectively,and prostate biopsies were conducted under the guidance of both.Taking pathological diagnosis as the gold standard,the diagnostic efficacy of conventional ultrasound and transrectal ultrasound elastography for prostate cancer and the positive rate of puncture under the guidance of both were compared.Results The sensitivity,specificity,and accuracy of transrectal ultrasound elastography in diagnosing prostate cancer were 91.67%,88.89%,and 90.00%,respectively,which were significantly higher than corresponding 62.50%,63.64%,and 63.33%of conventional ultrasound(all P<0.05).The positive rate of puncture point of transrectal ultrasound elastography was 52.17%,which was higher than 17.14%of conventional ultrasound(P<0.05).Conclusion Transrectal ultrasound elastography-guided prostate biopsy is of great value in the diagnosis of prostate cancer.Compared with conventional ultrasound,it can improve the detection rate of prostate cancer.
