摘　　要：我国人口基数大、人口老龄化以及卒中人群年轻化日渐严重,在急性脑卒中治愈率及生存率提高的同时,高致残率及高复发率成为脑卒中人群二级预防的重点问题。脑卒中二级预防社区管理是脑卒中二级预防工作的延伸,在对住院的脑卒中患者进行及时有效救治、危险因素管理的同时,不能忽略卒中患者回到社区后其可控危险因素的管理。本文针对脑卒中二级预防的社区管理模式及其应用效果给予综述。The problems of large population base,aging population,and rejuvenation of stroke populations in China are becoming more serious.While the cure rate and survival rate of acute stroke are increasing,high disability and recurrence rates have become key issues for secondary prevention of stroke population.Community management of secondary stroke prevention is an extension of secondary stroke prevention.While timely and effective treatment and risk factor management are provided to hospitalized stroke patients,the management of controllable risk factors after stroke patients return to the community can not be ignored.The community management model of secondary prevention of stroke and its effects are summarized as follows.
