摘　　要：目的探讨晚期胃癌患者一线化疗前后的T细胞免疫功能及营养状况的变化。方法分别检测80例晚期胃癌患者一线化疗前和化疗3个月后外周血T淋巴细胞亚群水平,同时进行主观全面评价法(PG-SGA)评分。对比不同疗效组T细胞亚群水平、PG-SGA评分及其化疗前后的变化,并分析T细胞亚群水平与PG-SGA评分两者间的相关性。结果在化疗前、化疗后,CR+PR组患者外周血T细胞亚群水平均高于SD组、PD组,而PG-SGA评分均低于SD组、PD组(P<0.05)。经化疗后患者外周血中各T淋巴细胞亚群水平发生不同程度的降低,而PG-SGA评分较化疗前升高。化疗前或化疗后,PG-SGA评分与外周血CD3^(+)、CD4^(+)、CD8^(+)T淋巴细胞水平均呈负相关(P<0.05)。结论胃癌晚期患者经一线化疗后其免疫功能及营养状况会发生不同程度的改变,且两者密切相关。Objective To investigate the changes of T cell immune function and nutritional status in advanced gastric cancer patients before and after first-line chemotherapy.Methods T lymphocyte subsets in peripheral blood of 80 patients with advanced gastric cancer were detected before first-line chemotherapy and 3 months after chemotherapy,and PG-SGA score was performed at the same time.The levels of T cell subsets,PG-SGA score and the changes before and after chemotherapy in different therapeutic groups were compared,and the correlation between T cell subsets and PG-SGA score was analyzed.Results Before and after chemotherapy,the levels of T lymphocyte subsets in peripheral blood in CR+PR group were higher than those in SD group and PD group,while PG-SGA scores were lower than those in SD group and PD group(P<0.05).After chemotherapy,the levels of T lymphocyte subsets in peripheral blood of patients decreased in different degrees,while the PG-SGA score was higher than that before chemotherapy.Before or after chemotherapy,PG-SGA score was negatively correlated with the levels of CD3^(+),CD4^(+),CD8^(+)T lymphocyte in peripheral blood(P<0.05).Conclusion Immune function and nutrition of advanced gastric cancer patients after first-line chemotherapy will change in different degrees,and they are closely related.
