摘  要:热消融是一种局部治疗肿瘤的新型技术,具有非侵入性的特点,已经被越来越多地应用于治疗原发性和转移性癌症上。热消融,如高强度聚焦超声、微波消融、射频消融等,能通过产生局部物理高温导致肿瘤细胞变性坏死。热消融不仅能够原位灭活肿瘤,还可以促进相关肿瘤抗原的释放,启动宿主的抗肿瘤免疫反应。该文从抗原和分泌性免疫物质的角度总结了热消融后抗肿瘤免疫产生和增强的机制。由于单一热消融治疗可引起的机体抗肿瘤免疫能力有限,该文还总结了热消融联合包括免疫药物、检查点抑制剂和化疗等在内的不同免疫治疗的临床应用,强调了免疫联合治疗可以进一步提高机体的抗肿瘤免疫能力,更有效地治疗肿瘤,有良好的应用前景。Thermal ablation is a new type of minimally invasive treatment for primary and metastatic tumors.Thermal ablation,such as high-intensity focused ultrasound,microwave ablation and radiofrequency ablation,causes the degeneration and necrosis of tumor cells and tissues by local physical hyperthermia.Thermal ablation not only inactivates tumors in situ,but also promotes the release of relevant tumor antigens and initiates the host's antitumor immune response.The mechanisms of antitumor immunity generation and enhancement after thermal ablation from the perspectives of antigenic and secretory immune substances will be summarized in this article.Since the limitation of anti-tumor immunity induced by single thermal ablation,the clinical application of thermal ablation combined with different immunotherapy including immune drugs,checkpoint inhibitors and chemotherapy will be summarized as well in order to emphasize the enhancement of anti-tumor immunity by the combined therapy,which has good application prospects.
