结肠神经内分泌癌合并肺、肾上腺转移1例

摘　　要：目前大多数关于神经内分泌癌的文献主要来源于对小细胞肺癌的研究,而结肠神经内分泌癌的研究数据很少。本文报道了1例结肠神经内分泌癌合并肺、肾上腺转移病例,患者就诊时消化道症状表现不明显,主要表现为活动后气促等肺部不适症状,第1次肺组织病理检查结果倾向腺癌,而第2次肺组织病理检查结果提示神经内分泌癌,消化道来源可能性大。结合患者影像学检查结果及查阅文献资料,最终诊断为结肠神经内分泌癌Ⅳ期。It is reported that most of neuroendocrine carcinoma are in the bronchopulmonary system,while studies on colonic neuroendocrine carcinoma are limited. This article reported a case of colonic neuroendocrine carcinoma with lung and adrenal metastasis. The patient mainly manifested as shortness of breath after exercise, but the gastrointestinal symptoms were not obvious. The first histopathological examination tended to be lung adenocarcinoma, however, the second lung histopathological result suggested neuroendocrine cancer that originated from digestive tract. Combined with the results of the patient’s imaging examination and literature review, the final diagnosis was colonic neuroendocrine carcinoma stage Ⅳ.
