盐酸羟考酮片联合盐酸阿米替林片在前列腺癌骨转移合并重度癌痛患者中的应用

摘　　要：目的探讨盐酸羟考酮片联合盐酸阿米替林片在前列腺癌骨转移合并重度癌痛患者中的应用效果。方法选择2016年3月至2020年3月淮安市第一人民医院收治的前列腺癌骨转移合并重度癌痛患者78例为研究对象。依据随机数字表法将入选患者分为观察组和对照组,每组各39例。对照组患者口服盐酸羟考酮片,观察组患者在对照组基础上联合盐酸阿米替林片治疗。两组患者均持续治疗2周。比较两组患者疼痛缓解情况、治疗前后疼痛因子和生活质量变化以及不良反应发生情况。结果观察组患者疼痛明显缓解率显著高于对照组(P<0.05)。两组患者治疗后血清去甲肾上腺素(norepinephrine,NE)和5-羟色胺(5-hydroxytryptamine,5-HT)水平、生活质量量表各维度评分均显著低于本组治疗前(均P<0.05)。观察组患者治疗后血清NE和5-HT水平、生活质量量表各维度评分均显著低于对照组(均P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论盐酸羟考酮片联合盐酸阿米替林片在前列腺癌骨转移合并重度癌痛患者中应用效果良好,可缓解患者疼痛,抑制疼痛因子释放,改善患者生活质量。Objective To investigate the clinical effect of oxycodone hydrochloride tablets combined with amitriptyline hydrochloride tablets in patients with prostate cancer bone metastasis complicated with severe cancer pain.Method From March 2016 to March 2020,78 cases of patients with prostate cancer bone metastasis complicated with severe cancer pain admitted to Huai'an First People's Hospital were selected and randomly divided into observation group and control group,with 39 cases in each group.Control group patients were given oxycodone hydrochloride tablets,observation group patients were given amitriptyline hydrochloride tablets on the basis of control group.Both groups were treated for 2 weeks.Pain relief,pain factors and quality of life before and after treatment,and adverse reactions were compared between the two groups.Result The pain relief rate of patients in observation group was significantly higher than that in control group(P<0.05).After treatment,the serum norepinephrine(NE)and 5-hydroxytryptamine(5-HT)levels and scores of each dimension in the quality of life scale of patients in the two groups were significantly lower than those before treatment(all P<0.05).After treatment,the serum NE and 5-HT levels and scores of each dimension in the quality of life scale of patients in observation group were significantly lower than those in control group(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Oxycodone hydrochloride tablets combined with amitriptyline hydrochloride tablets has a good effect on patients with prostate cancer bone metastasis complicated with severe cancer pain,which can relieve pain,inhibit the release of pain factors and improve the quality of life of patients.
