摘　　要：目的:探讨前列腺癌(PCa)患者癌胚抗原(CEA)和前列腺特异性抗原(PSA)的表达与经直肠前列腺超声弹性成像征象的相关性。方法:选取2018年6月至2020年8月在秦皇岛市第一医院诊治的前列腺癌患者100例进行回顾性研究,采取所有患者的3ml清晨空腹静脉血,离心后提取血清,通过电化学发光免疫法,对CEA、PSA水平进行测定。经直肠前列腺超声弹性成像检查统计超声弹性成像Kamoi评分与弹性应变率比值(SR)。结果:100例前列腺癌患者CEA的平均水平为(31.2±13.5)ng/ml,PSA的平均水平为(24.8±6.7)ng/ml。100例前列腺癌患者病灶以前列腺外腺区分布为主,肿瘤最大径>1.0cm的病灶81个,其中Kamoi评分为5分者3个,Kamoi评分为4分者33个,Kamoi评分3分者45个;最大径≤1.0cm的病灶19个,Kamoi评分4分者15个,Kamoi评分3分者4个。Kamoi评分为4(3,5)分;SR比值为6.9~14.0,平均SR比值为(10.1±3.3)。PCa患者血清CEA、PSA水平与SR比值及Kamoi评分均呈显著正相关(P<0.05)。结论:前列腺癌患者血清CEA和PSA的表达水平与SR值及Kamoi评分呈显著正相关性,因此在分析前列腺癌患者超声弹性成像征象时可结合患者血清CEA和PSA等表达进行分析,以便更好地对病情进行早期诊断。Objective:To explore the correlation between the expression of carcinoembryonic antigen(CEA)and prostate specific antigen(PSA)in patients with prostate cancer(PCa)and the signs of transrectal ultrasound elastography of the prostate.Methods:100 patients with prostate cancer treated in Qinhuangdao first hospital from June 2018 to August 2020 were selected for retrospective study.3ml morning fasting venous blood of all patients was taken,and the serum was extracted after centrifugation.The levels of CEA and PSA were measured by electrochemiluminescence immunoassay.Transrectal ultrasound elastography of the prostate statistical ultrasound elastography kamoi score and elastic strain rate ratio(SR).Results:The average level of CEA in 100 prostate cancer patients was(31.2±13.5)ng/ml,and the average level of PSA was(24.8±6.7)ng/ml.In 100 cases of prostate cancer,the lesions were mainly distributed in the extra-prostatic glands.There were 81 lesions with a tumor greater than 1.0 cm in diameter,3 of which had a Kamoi score of 5,33with a Kamoi score of 4,and 45 with a Kamoi score of 3;There were 19 lesions with the largest diameter≤1.0 cm,15 with Kamoi score of 4,and 4 with Kamoi score of 3.The Kamoi score was 4(3,5)points;the SR ratio was 6.9 to 14.0,and the average SR ratio was(10.1±3.3).Serum CEA and PSA levels in PCa patients were significantly positively correlated with SR ratio and Kamoi score(P<0.05).Conclusion:The expression levels of serum CEA and PSA in prostate cancer patients were significantly positively correlated with SR value and Kamoi score.Therefore,the expression levels of serum CEA and PSA in prostate cancer patients could be combined with the analysis of ultrasonic elastography signs in order to better diagnose the disease early.
