白内障术后眼表损伤rb-bFGF滴眼液治疗的早期效果

摘　　要：目的探讨重组牛碱性成纤维细胞生长因子(rb-bFGF)滴眼液治疗年龄相关性白内障手术所致眼表损伤的早期效果。方法采用前瞻性研究。将2020年8月至11月于大连大学附属中山医院眼科行白内障超声乳化手术的60例(60只眼)纳入研究。按随机数字表法分为两组,各30例(30只眼):对照组术后应用普拉洛芬滴眼液滴眼4次/d,研究组在对照组的基础上联合应用rb-bFGF滴眼液滴眼4次/d。随访30 d,观察两组手术前后眼表损伤OSDI评分、BUT和SIt的差异。结果术后7 d,两组患者BUT与SIt均较术前减低,OSDI评分均较术前增高(P<0.05)。术后30 d:研究组OSDI评分低于对照组(F=4.770,P=0.033);研究组BUT、SIt明显高于对照组(F=53.156,19.324;P<0.001)。结论应用rb-bFGF滴眼液能有效减轻白内障手术所致眼表不适症状,改善泪液分泌量以及恢复泪膜稳定性,但对已存在的角膜上皮损伤治疗效果不明显,可能需要长期随访观察。Objective To investigate the efficacy of recombinant bovine basic fibroblast growth factor(rb-bFGF)eye drops in the treatment of ocular surface damage caused by age-related cataract surgery.Methods This was a prospective research.A totol of 60 eyes of 60 patients undergoing phacoemulsification for cataract from Aug.to Nov.2020 in Zhongshan Hospital of Dalian University were included in the study.All cases were randomly divided into two groups with 30 eyes of 30 cases in each group based on random number table methods:the control group was given pranoprofen eye drops after the operation,and rb-bFGF eye drops were additionally combined for the study group.The eye drops were given 4 times a day for each group.All cases were followed up for 30 days to observe the differences in the ocular surface disease index(OSDI)score,breakup time(BUT)and SchirmerⅠtest(SIt)of the ocular surface damage between the study group and the control group before and after operation.Results At 7 days after operation,BUT and SIt of the two groups were lower than those before operation,while OSDI scores were higher than those before operation(P<0.05).At 30 days after operation,the OSDI score of the study group was lower than that of the control group(F=4.770,P=0.033);the BUT and SIt of the study group were significantly higher than those of the control group(F=53.156,19.324;P<0.001).Conclusion The rb-bFGF eye drops can effectively alleviate the symptoms of ocular surface discomfort caused by cataract surgery,improve tear secretion and restore tear film stability,but it is not effective for the existing corneal epithelial injury,and long-term follow-up observation may be needed.
