开角型青光眼Ex-Press引流器植入术的疗效观察

摘　　要：目的评估Ex-Press引流器植入术治疗开角型青光眼的效果。方法回顾性病例对照研究。分析郑州市第二人民医院2016年5月至2017年4月开角型青光眼56例(72眼)的临床资料。患者分为两组,引流器组30例(46眼);小梁切除术组26例(26眼)。术后随访6~12个月。结果两组患者术后视力改善率差异无统计学意义(P>0.05)。眼压均较术前明显降低(P<0.05)。两组之间眼压比较,差异无统计学意义(P>0.05)。术后6个月,两组患者功能型滤泡形成率分别为90.00%和92.31%(P>0.05)。两组患者术后均未见严重并发症,但小梁切除术组术后低眼压、前房积血、浅前房及黄斑水肿等并发症的发生率为50.00%,明显高于引流器植入组的10.00%(P<0.05)。结论Ex-Press引流器植入术治疗开角型青光眼,疗效显著,术后并发症发生率低。Objective To evaluate the clinical efficacy of Ex-Press drainage implantation in the treatment of open angle glaucoma.Methods Retrospective case-control study.The clinical data of 72 eyes of 56 patients with open angle glaucoma from May 2016 to Apr.2017 in Zhengzhou Second People’s Hospital were retrospectively analyzed.All cases were divided into 2 groups.The Ex-Press drainage implantation group with 46 eyes of 30 cases and trabeculectomy group with 26 eyes of 26 cases.The followed up time was 6-12 months postoperatively.Results After surgery,the difference was not statistically significant in the visual improvement rate between the two groups(P>0.05).The intraocular pressure in the two groups was obviously lower than that before surgery(P<0.05).At 3 and 6 months after surgery,there was no significant difference in the intraocular pressure of patients between the two groups(P>0.05).At 6 months after surgery,the formation rate of functional bleb of patients in the Ex-Press drainage group and trabeculectomy group were 90.00%and 92.31%respectively(P>0.05).There was no severe complication occurred.However,the incidence of complication,such as low intraocular pressure,hyphema,shallow anterior chamber,and macular edema was 50.00%in the trabeculectomy group,which was obviously higher than 10.00%in the Ex-Press group(P<0.05).Conclusion The Ex-Press drainage implantation in the treatment of open-angle glaucoma is effective and its incidence of postoperative complications is low.
