摘  要:目的探讨平原居民赴高原地区工作的眼血流动力学的变化。方法对2017年8月15日至8月18日于日喀则人民医院超声科对从上海抵达日喀则1个月内的26名汉族男性进行双眼超声灰阶以及彩色多普勒超声血流探测,测定眼部动脉的血流动力学指标,并在1年后再次于同一地点测定。同时随机选取日喀则当地的藏族男性居民26名作为对照组。结果观察组的眼动脉、睫状后长动脉、视网膜中央动脉和颈内动脉的收缩期峰值流速均较对照组低(P<0.05),阻力指数均较对照组高(P<0.05)。观察组颈外动脉的收缩期峰值流速与对照组无显著差异,阻力指数高于对照组(P<0.05)。在高原居住1年并在一定程度吸氧后,观察组眼动脉、视网膜中央动脉与颈内动脉的收缩期峰值流速均有上升,阻力指数均有下降(P<0.05)。睫状后长动脉的收缩期峰值流速上升(P<0.05),阻力指数未见显著变化。颈外动脉的收缩期峰值流速未见显著变化,阻力指数下降(P<0.05)。结论高海拔环境工作导致汉族居民眼部动脉收缩期峰值流速降低,阻力指数增高,在一年工作生活后有所改善。Objective To investigate the change of ocular hemodynamics of flatland residents working in high altitude area.Methods Gray scale ultrasound and color Doppler flow detection were performed on both eyes of 26 males of Han nationality who arrived in Shigatse from Shanghai within one month from 15th to 18th,August 2017 in Shigatse People’s Hospital.The hemodynamic indexes of ophthalmic artery were measured again one year later at the same place.At the same time,26 Tibetan male residents in Shigatze were randomly selected as control group.Results The peak systolic velocity(PSV)of ophthalmic artery(OA),long posterior ciliary artery(LPCA),central retinal artery(CRA)and internal carotid artery(ICA)in the observation group were lower than those in the control group(P<0.05),and the resistance index(RI)was higher than that in the control group(P<0.05).There was no significant difference in PSV of external carotid artery(ECA)between the two groups,and RI was higher than that of the control group(P<0.05).After living at high altitude for one year with oxygen inhalation to a certain extent,PSV of OA,CRA and ICA of the observation group increased,and the RI decreased(P<0.05).PSV of the LPCA increased(P<0.05),RI did not change significantly.PSV of ECA did not change significantly,while RI decreased(P<0.05).Conclusion Peak systolic velocity of the ophthalmic arteries decreased and the resistance index increased in Han nationality working at high altitude environment,which improved after one year of living and work there.
