探讨磨牙大面积缺损治疗中应用纤维桩联合氧化锆全冠修复的临床效果

摘　　要：目的分析磨牙大面积缺损采用纤维桩联合氧化锆全冠修复治疗的效果。方法选取2017年5月—2020年1月期间于南京市溧水区人民医院就诊的磨牙大面积缺损患者共计84例,按照随机数字表法将其分两组,对照组42例,采用纤维桩修复,研究组42例,采用纤维桩联合氧化锆全冠修复。于治疗后3个月进行完整性、适应性、颜色匹配及美观度评估,并评估修复优良率;治疗后随访6个月,统计随访期间松动脱落、根折、继发龋齿等并发症发生情况。结果治疗后3个月,研究组修复完整性、边缘适应性及颜色匹配和解剖外形效果好的患者所占比例明显高于对照组,差异有统计学意义(P<0.05)。研究组修复优良率为97.62%,高于对照组的80.95%,差异有统计学意义(χ^(2)=4.480,P=0.034)。研究组生理疼痛、功能限制、食物嵌塞及咬合评分分别为(0.95±0.52)分、(0.86±0.17)分、(1.21±0.42)分、(1.02±0.31)分,较对照组的(1.56±0.68)分、(1.82±0.67)分、(2.05±0.59)分、(2.21±0.33)分低,差异有统计学意义(t=4.618、9.001、7.517、17.033,P<0.05)。随访6个月期间,研究组并发症总发生率为2.38%,较对照组的19.05%低,组间差异有统计学意义(χ^(2)=4.480,P=0.034)。结论磨牙大面积缺损患者应用纤维桩联合氧化锆全冠修复治疗,可达到较好的修复效果,且美观度较高,能够提高患者口腔健康生活质量,同时并发症发生率较低。Objective To analyze the effect of repairing large-area molar defects with fiber posts combined with zirconia crowns.Methods A total of 84 patients with large-area molar defects who were treated at the People's Hospital of Lishui District,Nanjing City from May 2017 to January 2020 were selected.They were divided into two groups according to the random number table method.The control group 42 cases were repaired with fiber posts,42 cases in the study group,using fiber posts combined with zirconia full crown repair.Completeness,adaptability,color matching and aesthetics were evaluated 3 months after treatment,and the rate of good repair was evaluated;follow-up for 6 months after treatment,statistics of the occurrence of complications such as loosening and shedding,root fracture,and secondary dental caries during the follow-up period.Results Three months after treatment,the proportion of patients with good repair integrity,edge adaptability,color matching and anatomical shape in the study group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The excellent and good rate of repair in the study group was 97.62%,which was higher than 80.95%in the control group,and the difference was statistically significant(χ^(2)=4.480,P=0.034).The scores of physiological pain,functional limitation,food impaction and occlusion in the study group were(0.95±0.52)points,(0.86±0.17)points,(1.21±0.42)points,and(1.02±0.31)points,respectively,which were higher than those of the control group(1.56±0.68)points,(1.82±0.67)points,(2.05±0.59)points,(2.21±0.33)points,and the difference was statistically significant(t=4.618,9.001,7.517,17.033,P<0.05).During the 6-month follow-up,the total incidence of complications in the study group was 2.38%,which was lower than that in the control group 19.05%,and the difference between the groups was statistically significant(χ^(2)=4.480,P=0.034).Conclusion The application of fiber post combined with zirconia full crown restoration treatm
