宫颈涂片联合阴道镜检查在宫颈疾病中的诊断价值及准确性

摘　　要：目的探讨在进行宫颈疾病的诊断时,将宫颈涂片联合阴道镜应用于患者诊断中的效果,探讨其临床可应用价值。方法选择2019年1—10月该院收治的中妇产科疑似宫颈疾病患者580例进行研究,该研究中所有患者在入院时被临床确诊为宫颈疾病,并且选择对应的方式进行治疗。患者分别应用宫颈涂片和阴道镜进行检查,对比两种检查方式的诊断准确率。结果580例患者经病理学检查显示,NILM、LSIL、HSIL、CA分别为518例(89.31%)、32例(5.52%)、28例(4.83%)、2例(0.34%);宫颈涂片、宫颈涂片与阴道镜检查及病理均以≥HSIL为检验诊断标准;580例患者宫颈涂片+阴道镜检查诊断灵敏度90.32%(56/62)、特异度99.61%(516/518)及准确率98.62%(572/580)高于宫颈涂片的53.23%(33/62)、93.82%(486/518)、89.48%(519/580),差异有统计学意义(χ^(2)=23.172、27.369、43.285,P<0.05)。结论在对宫颈疾病患者进行检查时,阴道镜以及宫颈涂片应用于宫颈疾病的诊断中,有助于使患者的诊断准确率得到提升,而为了保障最终的诊断效果,在对患者进行检查时,可以采用阴道镜以及宫颈涂片的方式进行联合应用,可以将其作为一种良好的检验方式。Objective To explore the effect of applying cervical smear combined with colposcopy in the diagnosis of patients in the diagnosis of cervical diseases,and to explore its clinical application value.Methods Selected 580 patients with suspected cervical disease in the Department of Obstetrics and Gynecology admitted to the hospital from January to October 2019 for the study.All patients in the study were clinically diagnosed with cervical disease at the time of admission,and selected corresponding methods for treatment.The patients were examined by cervical smear and colposcopy respectively,and the diagnostic accuracy of the two examination methods were compared.Results Pathological examinations of 580 patients showed that NILM,LSIL,HSIL,and CA were 518 cases(89.31%),32 cases(5.52%),28 cases(4.83%),and 2 cases(0.34%)respectively;cervical smear The diagnostic criteria for cervical smears,cervical smears,colposcopy and pathology were≥HSIL;the diagnostic sensitivity of cervical smears+colposcopy in 580 patients was 90.32%(56/62),and the specificity was 99.61%(516/518).And the accuracy rate of 98.62%(572/580)was higher than 53.23%(33/62),93.82%(486/518),89.48%(519/580)of cervical smear,the difference was statistically significant(χ^(2)=23.172,27.369,43.285,P<0.05).Conclusion In the examination of patients with cervical disease,the application of colposcopy and cervical smears in the diagnosis of cervical disease can help improve the accuracy of the patient’s diagnosis.In order to ensure the final diagnosis effect,the patient is examined at the same time,the combined application of colposcopy and cervical pictures can be used,which can be used as a good inspection method.
