摘  要:目的:观察机器人立体定向辅助(ROSA)系统引导下立体定向颅内电极植入治疗难治性癫痫的临床效果。方法:选取医院收治的80例难治性癫痫患者,按照随机数表法将其分为对照组和观察组,每组40例。对照组接受大脑半球离断术治疗,观察组接受ROSA系统引导下立体定向颅内电极植入治疗。比较两组的临床治疗效果、手术时间及术中出血量等手术指标,以及术后6个月癫痫发作次数及神经心理学指标改善情况。结果:观察组治疗总有效率为95.00%,高于对照组的75.00%,差异有统计学意义(x^(2)=4.804,P<0.05);两组手术时间、并发症发生率比较差异无统计学意义;观察组术中出血量少于对照组,差异有统计学意义(t=19.578,P<0.05)。两组术后6个月癫痫发作次数均少于术前,且观察组术后6个月癫痫发作次数少于对照组,差异有统计学意义(t=24.422,P<0.05)。观察组术后6个月总智商(FIQ)和语言智商(VIQ)评分高于术前,差异有统计学意义(t=2.911,t=2.692;P<0.05)。结论:ROSA引导下立体定向颅内电极植入治疗难治性癫痫临床效果优于大脑半球离断术,且癫痫发作次数、神经心理学指标改善效果更好。Objective:To observe the clinical effect of the robotic stereotactic assistance(ROSA)system-guided implantation of stereotactic intracranial electrode in the treatment of refractory epilepsy(RE).Methods:80 patients with RE who admitted to hospital were selected and were randomly divided into control group and observation group,with 40 patients in each group.The control group received cerebral hemispherectomy and the observation group received the treatment of ROSA system-guided implantation of stereotactic intracranial electrode.The clinical treatment effect,the time of operation,intraoperative blood loss and other surgical indicators,and the number of occurring epilepsy at the 6th month after surgery and the improvement of neuropsychological indicators were compared between the two groups.Results:The total effective rate of observation group was 95.00%,which was significantly higher than that(75.00%)of control group(x^(2)=4.804,P<0.05).There were no significant differences in the time of operation and the incidence of complication between the two groups.The intraoperative blood loss of observation group was less than that of control group(t=19.578,P<0.05).The number of occurring epilepsy both two groups at the 6th month after surgery were less than that before surgery.And the number of occurring epilepsy at the 6th month after surgery in observation group was significantly less than that in control group(t=24.422,P<0.05).The scores of full intelligence quotient(FIQ)and verbal intelligence quotient(VIQ)of observation group at the 6th month after surgery were significantly higher than those before surgery(t=2.911,t=2.692,P<0.05),respectively.Conclusion:The clinical effect of ROSA system-guided implantation of stereotactic intracranial electrode in the treatment of RE is better than that of cerebral hemispherectomy,and the number of occurring epilepsy and the improved effects of neuropsychological indicators of this method are better.
