复元活血汤治疗肝硬化疗效观察及对肝脏纤维化的影响

摘　　要：目的:观察复元活血汤治疗肝硬化的临床疗效及对肝功能和肝脏纤维化的影响。方法:选取74例肝硬化患者,随机分为对照组和研究组各37例。2组均使用常规保肝药物甘利欣片等治疗,对照组加用丹参注射液治疗,研究组加用复元活血汤治疗。治疗30 d后,观察比较2组临床疗效,2组治疗前后肝功能和肝脏纤维化指标变化。结果:研究组总有效率为94.59%,对照组为78.38%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组白蛋白(Alb)、谷丙转氨酶(ALT)、天门冬氨酸氨基转移酶(AST)及总胆红素(TBil)水平比较,差异无统计学意义(P>0.05);治疗后,2组Alb水平较治疗前升高(P<0.05),ALT、AST、TBil水平较治疗前降低(P<0.05),且研究组上述指标改善较对照组更显著(P<0.05)。治疗前,2组透明质酸(HA)、Ⅳ型胶原(Ⅳ-C)、层粘连蛋白(LN)、Ⅲ型前胶原(PCⅢ)水平比较,差异无统计学意义(P>0.05);治疗后,2组HA、LN、Ⅳ-C、PCⅢ水平均较治疗前降低(P<0.05),且研究组上述指标均低于对照组(P<0.05)。结论:在常规保肝治疗的基础上,应用复元活血汤治疗肝硬化,可提高临床疗效,有效改善患者肝功能及肝纤维化指标。Objective:To observe the clinical effect of Fuyuan Huoxue tang on liver cirrhosis and its effect on liver function and liver fibrosis.Methods:A total of 74 cases with liver cirrhosis were selected and randomly divided into the control group and the study group,37 cases in each group.Both groups were treated with routine liver-protection medicine,such as Ganlixin tablets(diammonium glycyrrhizinate tablets);the control group was additionally treated with Danshen injection,and the study group was additionally treated with Fuyuan Huoxue tang.After 30-day treatment,the clinical effect was observed and compared between the two groups;the changes in the liver function and indexes of liver fibrosis were observed and compared in the two groups before and after treatment.Results:The total effective rate was 94.59%in the study group,and78.38%in the control group,the difference being significant(P<0.05).Before treatment,when compared levels ofalbumin(Alb),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and total bilirubin(TBil)between the two groups,there was no significance in the difference(P>0.05).After treatment,levels of Alb in the two groups were increased when compared with those before treatment(P<0.05),and levels of ALT,AST,and TBil were decreased(P<0.05);the improvement of above indexes in the study group was more significant than that in the control group(P<0.05).Before treatment,when compared levels ofhyaluronic acid(HA),collagen typeⅣ(Ⅳ-C),laminin(LN),and procollagen typeⅢ(PCⅢ)between the two groups,there was no significance in the difference(P>0.05).After treatment,levels of HA,LN,Ⅳ-C,and PCⅢin the two groups were decreased when compared with those before treatment(P<0.05),and the above indexes in the study group were lower than those in the control group(P<0.05).Conclusion:Based on the routine treatment of liver protection,Fuyuan Huoxue tang for liver cirrhosis can improve clinical effect and effectively improve the liver function and indexes of liver fibrosis.
