摘  要:目的探讨弥散神经突方向离散度与密度成像(NODDI)模型参数对垂体大腺瘤增殖潜能术前评估的应用价值。方法前瞻性纳入16例垂体大腺瘤的磁共振弥散数据,并以此为基础拟合扩散张量成像(DTI)模型和NODDI模型并生成两种模型的参数图,然后计算肿瘤区域相应的直方图数值。Mann-Whitney U检验用于比较高Ki-67组(≥3%)和低Ki-67组(<3%)组间直方图数值的差异。结果年龄、性别、侵袭性评分及DTI模型直方图参数在两组间差异无统计学意义(P>0.05),而NODDI模型所得的神经密度(NDI)和各向同性间隔的体积分数(FISO)的方差在两组间差异有统计学意义(P<0.05)。结论 NODDI模型能有效评估垂体腺瘤的增殖潜能,有望为垂体腺瘤精准治疗提供客观依据。Objective To explore the value of NODDI in evaluating tumor proliferative potential of pituitary macroadenoma. Methods Sixteen patients with surgically proved pituitary macroadenoma were included in this study.MRI diffusion-weight data were fitted to DTI and NODDI model, respectively.In addition, parametric maps for both models were established, and then their corresponding histogram metrics for the tumor regions were calculated.Finally, the histogram metrics were compared between the groups of high Ki-67(≥3%) and that of low Ki-67(<3%) by using Mann-Whitney U test. Results There was no significant differencebetween the group of high Ki-67 and that of low Ki-67 in age, gender, aggressive score and DTI metrics(P> 0.05).Whereas, variance of NDI and FISO obtained by the NODDI model were statistically different between two groups(P< 0.01). Conclusion NODDI model can effectively assess the proliferative potential of pituitary macroadenomas and is expected to offer an objective evidence for precise treatment of pituitary macroadenoma.
