摘　　要：目的探讨子宫静脉内平滑肌瘤病的临床病理特点、诊治及预后。方法回顾性分析我院27例经手术病理确诊的子宫静脉内平滑肌瘤病患者的临床资料。结果患者平均年龄(45.21±8.72)岁,10例(37.04%)主要表现为月经改变,12例(44.44%)无明显的临床症状,仅在超声检查或妇科检查时发现。术前无确诊病例,所有患者均由术后病理证实。单纯病灶切除2例(7.41%),行全子宫切除或全子宫+一侧或双附件切除术25例(92.59%)。术后5例失访,22例患者平均随访(15.07±19.72)个月,均未复发。结论子宫静脉内平滑肌瘤病临床表现无特异性,术前误诊率高,诊断主要依靠病理,手术治疗以彻底切除病灶为主,术后加强随访,有利于改善预后。Objective To investigate the clinicopathological features,diagnosis,treatment and prognosis of uterine venous leiomyomatosis.Methods The clinical data of 27 cases with IVL diagnosed by surgical pathology were analyzed retrospectively.Results The average age of the patients was(45.21±8.72)years old.Ten cases(37.04%)mainly showed menstrual changes,and 12 cases(44.44%)had no obvious clinical symptoms,which were only found during ultrasonography or gynecological examination.There were no confirmed cases before operation,and all patients were confirmed by postoperative pathology.Simple focus resection was performed in 2 cases(7.41%),total hysterectomy or total hysterectomy+one side or double appendectomy in 25 cases(92.59%).Five patients were lost to follow-up,and 22 patients were followed up for an average of(15.07±19.72)months,there was no recurrence.Conclusion The clinical manifestations of uterine venous leiomyomatosis are nonspecific,the preoperative misdiagnosis rate is high,the diagnosis mainly depends on pathology,the surgical treatment is mainly to completely remove the focus,and the postoperative follow-up is helpful to improve the prognosis.
