改良式眼轮匝肌缩短术治疗下睑内翻合并眼睑松弛的短期效果观察

摘　　要：目的探讨改良式眼轮匝肌缩短术治疗下睑内翻合并眼睑松弛的短期效果。方法抽取2016年2月至2019年7月河南科技大学第一附属医院诊断为下睑内翻合并眼睑松弛患者72例(81眼),按手术方案分为传统组36例(39眼)与改良组36例(42眼)。传统组采用单纯眼轮匝肌缩短术,改良组采用改良式眼轮匝肌缩短术。比较两组手术效果、眼部症状评分、美学指标、并发症及复发情况。结果改良组总有效率为97.62%(41/42),高于传统组的82.05%(32/39),P<0.05。术后3个月,改良组眼部畏光、刺激感、异物感评分低于传统组(t=18.586、9.478、15.696,P<0.05),改良组睑裂宽度短于传统组(t=6.383,P<0.05),睑裂角、睑裂倾斜度大于传统组(t=3.576、13.409,P<0.05)。改良组并发症发生率低于传统组,χ^(2)=3.896,P=0.048。改良组复发率为2.50%(1/40),低于传统组的15.63%(5/32),但P>0.05。结论改良式眼轮匝肌缩短术治疗下睑内翻合并眼睑松弛短期效果良好,可改善临床症状和眼部美学指标,且并发症少。Objective To analyze the short-term effect of modified orbicularis oculi muscle shortener in the treatment of lower eyelid entropion with eyelid relaxation.Methods A total of 72 patients(81 eyes)with lower eyelid entropion and eyelid relaxation diagnosed in the First Affiliated Hospital of Henan University of Science and Technology from February 2016 to July 2019 were selected.they were divided into 36 cases(39 eyes)in the traditional group and 36 cases(42 eyes)in the modified group according to the surgical plan.The traditional group used simple orbicular oculi muscle shortening,and the modified group used modified orbicular oculi muscle shortening.The surgical effects,eye symptom scores,aesthetic indicators,complications and recurrence were compared between the two groups.Results The total effective rate of the modified group was 97.62%(41/42),which was higher than 82.05%(32/39)of the traditional group,P<0.05.Three months after operation,the scores of photophobia,irritation and foreign body sensation in the eyes of the modified group were lower than those of the traditional group(t=18.586,9.478,15.696,P<0.05).The eyelid cleft width of the modified group was shorter than that of the traditional group(t=6.383,P<0.05),the eyelid cleft angle and the eyelid cleft slope of the modified group were greater than those of the traditional group(t=3.576,13.409,P<0.05).The complication rate of the modified group was lower than that of the traditional group,χ^(2)=3.896,P=0.048.The recurrence rate of the modified group was 2.50%(1/40),which was lower than the 15.63%(5/32)of the traditional group,but P>0.05.Conclusions The modified orbicularis oculi muscle shortening has a good short-term effect in the treatment of lower eyelid inversion with eyelid relaxation,which can improve clinical symptoms and ocular aesthetics,with fewer complications.
