摘　　要：目的总结15例咽旁隙肿瘤的临床诊治经验,为选择手术适应证、减少手术并发症提供借鉴。方法15例咽旁隙肿瘤经增强CT和MRI等术前检查予以充分评估后,采用经口径路手术切除5例、经颈侧径路切除6例、经颈侧腮腺径路切除4例。结果所有患者肿瘤均成功切除,1例术后即刻出现颅底大出血,用明胶海绵等压迫填塞后成功止血;3例恶性肿瘤患者中2例患者术后接受放疗,1例失访,其余患者随访1.5~5年,均无复发。结论根据肿瘤部位,结合患者需求选择合适手术方式,能彻底切除肿瘤,术前充分的影像学评估对制定手术方案和预防手术并发症至关重要。Objective To summarize the clinical diagnosis and treatment of 15 cases of parapharyngeal space tumors,and provide reference for further selection of surgical indications and reduction of surgical complications.Methods The fifteen cases of parapharyngeal space tumors were fully evaluated by enhanced CT and MRI,and other preoperative examinations.Who were performed a resection by oral approach in 5 cases,by cervical lateral approach in 6 cases,and by cervical parotid gland approach in 4 cases.Results The tumors were successfully removed in all patients.One case of them had massive hemorrhage in the skull base immediately after tumor resection,and hemostasis was successfully stopped by packing with gelatin sponge.Two cases of 3 patients with malignant tumors received radiotherapy after surgery,but one patient of them was lost to follow-up.The other patients without recurrence were followed up for 1.5 to 5 years.Conclusions According to the location of the tumors and the patient’s needs,the appropriate surgical method can be selected to completely remove the tumors.Preoperative adequate imaging evaluation is essential for formulating surgical plans and preventing surgical complications.
