三山参芪汤联合预激HCG方案治疗老年急性髓系白血病临床观察

摘　　要：目的探讨三山参芪汤联合预激HCG方案治疗老年急性髓系白血病的临床有效性及不良反应。方法选取2017年1月—2020年6月就诊的老年急性髓系白血病患者104例,随机分为2组,对照组使用预激HCG方案治疗,研究组使用三山参芪汤联合预激HCG方案治疗。比较2组心脏毒副反应、临床有效性、血制品输注量、造血恢复时间、不良反应。结果研究组II度、I度心脏毒副反应均显著低于对照组,0度心脏毒副反应显著高于对照组(P<0.05);研究组总有效率高于对照组(P<0.05);研究组血小板、红细胞血制品输注量少于对照组,血小板、白细胞造血恢复时间短于对照组(P<0.05);研究组肾损害、肝损害、电解质紊乱、感染、胃肠道反应的不良反应发生率均低于对照组(P<0.05)。结论三山参芪汤联合预激HCG方案治疗老年急性髓系白血病,患者血制品血小板、悬浮红细胞的输注量显著减少,加速骨髓造血恢复,肝肾损害、感染、胃肠道反应、心脏毒副反应有明显减少,治疗有效性显著提高,有助于改善患者预后,值得临床推广。Objective To investigate the clinical efficacy and adverse reactions of Sanshan Shenqi decoction combined with pre excitation HCG regimen in the treatment of elderly patients with acute myeloid leukemia.Methods 104 elderly patients with acute myeloid leukemia from January 2017 to June 2020 were selected and randomly divided into two groups.The control group was treated with pre excitation HCG regimen,and the study group was treated with Sanshan Shenqi decoction combined with pre excitation HCG regimen.Cardiac toxicity,clinical efficacy,blood transfusion volume,hematopoiesis recovery time and adverse reactions were compared between the two groups.Results The total effective rate of the study group was higher than that of the control group(P<0.05).The transfusion amount of platelet and red blood cell blood products in the study group was less than that in the control group,and the recovery time of platelet and leukocyte hematopoiesis in the study group was shorter than that in the control group(P<0.05).The renal damage,liver damage,liver damage and the recovery time of platelet and leukocyte hematopoiesis in the study group were shorter than those in the control group(P<0.05)The incidence of electrolyte disturbance,infection and gastrointestinal reaction was lower than that of the control group(P<0.05).Conclusion Sanshan Shenqi decoction combined with pre excitation HCG regimen in the treatment of elderly acute myeloid leukemia can significantly reduce the transfusion volume of blood platelet and suspended red blood cells,accelerate the recovery of bone marrow hematopoiesis,significantly reduce liver and kidney damage,infection,gastrointestinal reaction,cardiac toxicity,and improve the treatment effectiveness,which is helpful to improve the prognosis of patients and is worthy of clinical promotion.
