摘　　要：目的系统评价进展期胆囊癌新辅助治疗的临床疗效及安全性。方法采用Cochrane系统评价方法,计算机检索PubMed、Embase、Cochrane Library、Web of science、中国生物医学文献数据库(CBMdisc)、中国知网、万方数据库、维普数据库,根据纳入和排除标准选取文献,进行质量评价后采用Stata15.1软件进行meta分析。结果共纳入6项临床研究,362例研究对象。新辅助治疗后临床获益率为73%(95%CI:0.68~0.78,P<0.01)、手术切除率为36%(95%CI:0.21~0.52,P<0.01)、R0切除率96%(95%CI:0.90~0.99,P<0.01)、术后并发症发生率为17%(95%CI:0.02~0.40,P=0.01)、手术组与非手术组生存期均值差为29.51个月(95%CI:22.5~36.45,P<0.01),差异均有统计学意义。结论新辅助治疗对进展期胆囊癌手术切除患者临床疗效确切,且安全性良好。Objective To systematically evaluate the clinical efficacy and safety of neoadjuvant therapy for the advanced gallbladder cancer.Methods The method of Cochrane systematic review was used to search PubMed,Embase,Cochrane Library,Web of Science,China Biology Medicine disc(CBMdisc),China National Knowledge Infrastructure(CNKI),Wanfang database,and VIP database.The literatures were selected according to the inclusion and exclusion criteria.After quality assessment,Stata 15.1 software was used for the meta-analysis.Results A total of six clinical studies with 362 subjects were included.After neoadjuvant therapy,the clinical benefit rate was 73%(95%CI:0.68 to 0.78,P<0.01),the resection rate was 36%(95%CI:0.21-0.52,P<0.01),the R0 resection rate was 96%(95%CI:0.90 to 0.99,P<0.01)and the surgical complication rate was 17%(95%CI:0.02 to 0.40,P=0.01).The mean difference in the survival time between the surgery group and the non-surgery group was 29.51 months(95%CI:22.5 to 36.45,P<0.01),and the differences were statistically significant.Conclusion The neoadjuvant therapy is effective and safe for patients undergoing surgical resection of advanced gallbladder cancer.
