摘 要:目的探讨血清淀粉酶、胰淀粉酶、胰蛋白酶原激活肽(TAP)联合检测应用于急性胰腺炎(AP)患者早期诊断的意义。方法选取济宁医学院附属高唐县人民医院在2019年2月至2020年2月确诊并治疗的55例AP患者,作为研究组,年龄(40.6±1.8)岁,男32例,女23例;选取同期行健康体检的健康人群55例,作为对照组,年龄(40.8±1.6)岁,男33例,女22例。对两组人群检测血清淀粉酶、胰淀粉酶及TAP水平。观察比较研究组治疗前后与对照组的血清淀粉酶、胰淀粉酶及TAP水平。结果对照组血清淀粉酶水平为(82.1±25.5)U/L、胰淀粉酶水平为(75.8±8.2)U/L、TAP水平为(9.8±2.7)nmol/L;研究组治疗前血淀粉酶水平为(846.3±146.9)U/L、胰淀粉酶水平为(796.3±96.7)U/L、TAP水平为(1.3±0.6)nmol/L,治疗后血清淀粉酶水平为(96.9±65.8)U/L、胰淀粉酶水平为(89.1±28.3)U/L、TAP水平为(7.3±1.8)nmol/L;研究组治疗前血清淀粉酶、胰淀粉酶均显著高于对照组(均P<0.05),TAP水平低对照组(P<0.05);研究组治疗后血清淀粉酶、胰淀粉酶水平均显著低于治疗前(均P<0.05),血清TAP水平高于治疗前(P<0.05)。结论AP患者体内的血清淀粉酶、胰淀粉酶及TAP水平均显著性的升高,可通过检测患者的血淀粉酶、胰淀粉酶及TAP水平来辅助临床医生更好地诊断AP。Objective To explore the significance of blood amylase,pancreatic amylase,and trypsinogen activation peptide(TAP)in the early diagnosis of patients with acute pancreatitis(AP).Methods Fifty-five(40.6±1.8)years old patients with AP diagnosed and treated in Gaotang County People's Hospital,Jining Medical College from February 2019 to February 2020 were selected as a study group,including 32 males and 23 females.Fifty-five(40.8±1.6)years old healthy people who had physical examination in Gaotang County People's Hospital,Jining Medical College during the same period were selected as a control group,including 33 males and 22 females.The levels of blood amylase,pancreatic amylase,and TAP in both groups were detected and were compared between these two groups and between before and after the treatment in the study group.Results The levels of blood amylase,pancreatic amylase,and TAP were(82.1±25.5)U/L,(75.8±8.2)U/L,and(9.8±2.7)nmol/L in the control group,were(846.3±146.9)U/L,(796.3±96.7)U/L,and(1.3±0.6)nmol/L in the study group before the treatment,and were(96.9±65.8)U/L,(89.1±28.3)U/L,and(7.3±1.8)nmol/L in the study group after the treatment,with statistical differences between the control group and the study group before the treatment and between before and after the treatment in the study group(all P<0.05).Conclusion The levels of blood amylase,pancreatic amylase,and TAP are significantly increased in patients with AP,so the detection of their levels can help clinicians to better diagnose AP.
