引导骨再生术在前牙区种植术中的应用

摘　　要：目的探讨引导骨再生术在前牙区种植术中的临床应用价值。方法选择2018年2月—2020年7月医院收治行前牙种植术患者80例为研究对象。按照随机数字表法分为两组,各40例。观察组采用引导骨再生术,对照组仅采用将黏膜翻瓣并暴露牙槽棘顶位置,后置入愈合帽后缝合创面的方法。比较两组术后3个月牙槽骨密度、牙周探诊深度、临床附着丧失及龈退缩情况,两组术后3个月牙龈美学指标;比较两组种植效果、两组治疗期间发生的并发症。结果术后观察组牙槽骨密度高于对照组(P<0.05),牙周探诊深度小于对照组(P<0.05),临床附着丧失指数低于对照组(P<0.05),龈退缩小于对照组(P<0.05),术后3个月随访,观察组牙龈美学指标中白色美学指数和红色美学指数得分均显著高于对照组(P<0.05),观察组种植成功比例显著高于对照组(P<0.05),失败率显著低于对照组(P<0.05),观察组发生充血、感染、面部肿胀及探诊出血的比例显著低于对照组(P<0.05)。结论针对前牙缺损患者,实施引导骨再生术,能有效的提高种植牙稳定性,改善美学指标,提高临床治疗效果。Objective To explore the clinical value of guided bone regeneration in anterior teeth implantation.Methods 80 cases with anterior teeth implantation from February 2018 to July 2020 in our hospital were selected as research objects.According to the random number table method,they were divided into two groups,each with 40 cases.The observation group was guided bone regeneration,while the control group only was turned the mucosa flap and exposed the top of alveolar spine,then placed the healing cap and sutured the wound.The bone mineral density,periodontal probing depth,clinical attachment loss and gingival recession of the two groups 3 months after operation were compared,and the gingival aesthetic indexes 3 months after operation of the two groups were compared.Compared the implant effect and complications between the two groups during treatment.Results The postoperative alveolar bone density of the observation group was higher than that of the control group(P<0.05),the depth of periodontal exploration was smaller than that of the control group(P<0.05),the clinical attachment loss index was lower than that of the control group(P<0.05),and gingival recession decreased compared with the control group(P<0.05),follow-up 3 months after surgery,the white aesthetic index and red aesthetic index scores of the observation group’s gingival aesthetics index were significantly higher than those of the control group(P<0.05),and the success rate of implantation in the observation group was significantly higher than that of the control group(P<0.05),the failure rate was significantly lower than that of the control group(P<0.05).The rate of congestion,infection,facial swelling and bleeding during probing in the observation group was significantly lower than that of the control group(P<0.05).Conclusion Guided bone regeneration for patients with anterior tooth defect can effectively improve the stability of dental implants,improve the aesthetic index and improve the clinical treatment effect.
