血清高迁移率族蛋白A2、糖类抗原125、人附睾蛋白4在卵巢上皮癌中的表达及诊断价值分析

摘　　要：目的探讨血清高迁移率族蛋白A2(HMGA2)、糖类抗原125(CA125)、人附睾蛋白4(HE4)对卵巢上皮癌的诊断价值。方法将50例卵巢上皮癌患者作为恶性组,30例良性卵巢肿瘤患者作为良性组,30例良性妇科疾病患者作为对照组,比较3组患者血清HMGA2、CA125、HE4水平及不同病理类型卵巢上皮癌患者HMGA2、CA125、HE4水平,采用受试者工作特征(ROC)曲线分析HMGA2、CA125、HE4诊断卵巢上皮癌的临床价值。结果恶性组患者血清HMGA2、CA125、HE4水平均高于良性组和对照组,差异均有统计学意义(P﹤0.05)。浆液性癌及黏液性癌患者血清CA125、HE4水平均高于子宫内膜样癌患者,且浆液性癌患者血清CA125水平高于黏液性癌患者,差异均有统计学意义(P﹤0.05)。HMGA2、CA125、HE4联合诊断卵巢上皮癌的曲线下面积(AUC)为0.995,大于三者单独诊断的AUC(0.925、0.944、0.936)。结论HMGA2联合CA125、HE4诊断卵巢上皮癌能够显著提高临床诊断价值,能够作为卵巢癌的重要辅助诊断指标。Objective To investigate the diagnostic value of serum high mobility group A2(HMGA2),carbohydrate antigen 125(CA125),and human epididymal protein 4(HE4)in the ovarian epithelial carcinoma.Method Fifty patients with ovarian epithelial carcinoma were selected as the malignant group,30 patients with benign ovarian tumors were divided as the benign group,and 30 patients with benign gynecological diseases were regarded as the control group.The serum HMGA2,CA125,HE4 levels of the three groups of patients and the HMGA2,CA125,HE4 levels of patients with different pathological types of ovarian epithelial carcinoma were compared.The receiver operating characteristic(ROC)curve was drawn to visualize the clinical value of HMGA2,CA125,HE4 in the diagnosis of epithelial ovarian cancer.Result The serum HMGA2,CA125,and HE4 levels of the malignant group were higher than those of the benign group and the control group,and the differences were statistically significant(P<0.05).Serum CA125 and HE4 levels in patients with serous carcinoma and mucinous carcinoma were higher than those with endometrioid carcinoma,and serum CA125 levels in patients with serous carcinoma were higher than those with mucinous carcinoma,the differences were statistically significant(P<0.05).The area under the curve(AUC)of the combined diagnosis of HMGA2,CA125,and HE4 for ovarian epithelial carcinoma was 0.995,which was greater than the AUC diagnosed by the three indicators alone(0.925,0.944,0.936,respectively).Conclusion The HMGA2 detection combined with CA125 and HE4 could significantly improve the clinical diagnostic value in ovarian epithelial carcinoma,indicating whose result could be used as an important auxiliary diagnostic index of ovarian cancer.
