摘 要:目的探讨"改良的佩顿四步法"用于提高中医医院住院医师规范化培训中的急救技能培训的效果及可行性。方法选择94名在广东省中医院急诊科轮训并参加了出科前急救技能考核的中医规培医师作为改良的佩顿四步法组;同时,根据该组成员基本信息,按照1∶1选取94名同时期规培医师作为传统教学组。对两组的考核评分以及客观评价情况进行比较。结果考核得分方面,改良佩顿四步法组在除颤术、洗胃术及针刺急救术操作的对比中明显优于传统教学组(均P<0.05)。在客观评价上,两组在心肺复苏术(CPR)的按压效果、通气效果、胸外按压比例方面差异无统计学意义(均P>0.05);改良佩顿四步法组止血包扎术总用时短于传统教学组(P<0.05)。结论在中医规培急救技能培训上应用改良的佩顿四步法对于流程性的简单技能培训效果显著;而要提升高级技能的培训效果,需要进一步的强化练习。Objective:To explore the effect and feasibility of the modified Peyton′s Four-Step Approach in the standardized training of resident doctors in Chinese medicine hospitals.Methods:A total of 94 TCM doctors who were trained in the emergency department of Guangdong Provincial Hospital of Traditional Chinese Medicine and participated in the first aid skills assessment were selected as the modified Peyton′s Four-Step Approach group;at the same time,according to the basic information of the group members and in a ratio of 1∶1,another 94 students who received regular training were selected as the traditional teaching group.The assessment scores and objective evaluations of the two groups were compared.Results:In terms of assessment scores,the modified Peyton's Four-Step Approach group was significantly better than the traditional teaching group in the comparison of defibrillation,gastric lavage and acupuncture emergency operations(all P<0.05).In objective evaluation,there was no statistical difference between the two groups in CPR compression effect,ventilation effect,and chest compression ratio(all P>0.05);the total time of hemostatic bandaging in the modified Peyton′s Four-Step Approach group was shorter than that of traditional teaching group with statistical difference(P<0.05).Conclusion:The application of the modified Peyton′s Four-Step Approach to the training of traditional Chinese medicine regulations and first aid skills has a significant effect on the process of simple skills training;to improve the training effect of advanced skills,further intensive exercises are needed.
