摘 要:目的:探讨食管鳞癌患者外周血T淋巴细胞亚群以及红细胞免疫状态的变化,以期为临床诊疗和决策提供参考。方法:以本院2015年4月-2019年12月收治的100例食管鳞癌患者作为观察组,并选取同期在本院体检科体检的100例健康人作为对照组。检测并比较观察组和对照组、观察组术前和术后14 d、观察组术前不同病理分期患者的T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))和红细胞免疫相关指标[红细胞膜C3b受体花环形成率(RBC-C3bRR)、红细胞膜吸附免疫复合物花环形成率(RBC-ICR)、体循环免疫复合物花环形成率(CIC)]水平。结果:观察组患者术前CD3^(+)、CD4^(+)均低于对照组,而CD8^(+)水平高于对照组(P<0.05);与术前相比,观察组患者术后14 d外周血CD3^(+)、CD4^(+)水平均明显升高,CD8^(+)水平明显降低(P<0.05)。观察组患者术前RBC-C3bRR低于对照组,而RBC-ICR和CIC水平均高于对照组(P<0.05);与术前相比,观察组患者术后14 d的RBC-C3bRR水平明显升高,RBC-ICR和CIC水平均明显降低(P<0.05)。食管鳞癌Ⅲ期患者CD3^(+)和CD4^(+)水平均低于Ⅱ期和Ⅰ期,CD8^(+)水平高于Ⅱ期和Ⅰ期(P<0.05);食管鳞癌Ⅱ期患者CD3^(+)和CD4^(+)水平均低于Ⅰ期,CD8^(+)水平高于Ⅰ期(P<0.05)。食管鳞癌Ⅲ期患者RBC-C3bRR低于Ⅱ期和Ⅰ期,而RBC-ICR和CIC水平均高于Ⅱ期和Ⅰ期(P<0.05);食管鳞癌患者Ⅱ期RBC-C3bRR低于Ⅰ期,而RBC-ICR和CIC水平均高于Ⅰ期(P<0.05)。结论:食管鳞癌患者免疫功能下降,且下降趋势与病情严重程度相关,红细胞免疫功能和T淋巴细胞亚群检测可作为临床辅助诊断的一项重要免疫学参考依据。Objective:To investigate the changes of T lymphocyte subsets and erythrocyte immune status in peripheral blood of patients with esophageal squamous cell carcinoma,so as to provide reference for clinical diagnosis,treatment and decision-making.Method:A total of 100 patients with esophageal squamous cell carcinoma admitted to our hospital from April 2015 to December 2019 were taken as the observation group,and 100 healthy people who underwent physical examination in the physical examination department of our hospital during the same period were selected as the control group.T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+))and erythrocyte immune related indexes(RBC-C3bRR,RBC-ICR,CIC)were detected and compared between the observation group and the control group,the observation group before and 14 d after operation,and the observation group with different pathological stages before operation.Result:The levels of CD3^(+),CD4^(+) in the observation group were lower than those in the control group,while the levels of CD8^(+) in the observation group was higher than that in the control group(P<0.05);compared with before operation,the levels of CD3^(+)and CD4^(+) in peripheral blood of the observation group were significantly increased,while the level of CD8^(+) was significantly decreased 14 d after operation(P<0.05).The preoperative RBC-C3bRR of the observation group was lower than that of the control group,while the levels of RBC-ICR and CIC of the observation group were higher than those of the control group(P<0.05);compared with before operation,the level of RBC-C3bRR in the observation group was significantly higher,and the levels of RBC-ICR and CIC were significantly lower 14 d after operation(P<0.05).The levels of CD3^(+) and CD4^(+) in stage Ⅲ were lower than those in stage Ⅱ and Ⅰ,and the level of CD8^(+) was higher than those in stage Ⅱ and Ⅰ(P<0.05);the levels of CD3^(+) and CD4^(+) in stageⅡwere lower than those in stage Ⅰ,and the level of CD8^(+) was higher than that in stage Ⅰ(P<0.05).RBC-C3bRR i
