血清对称二甲基精氨酸水平对肾囊腺瘤的诊断及预后价值分析

摘　　要：目的:探究血清对称二甲基精氨酸(SDMA)水平对肾囊腺瘤的诊断及预后价值。方法:选择2017年1月-2020年1月于本院接受治疗的60例肾囊腺瘤患者为研究组,另选取同期于本院接受治疗的50例肾脏良性病变患者为对照组。检测两组的SDMA和甲胎蛋白(AFP)、癌胚抗原(CEA)水平,将研究组患者按照TNM分期分为Ⅰ、Ⅱ期(n=37)和Ⅲ、Ⅳ期(n=23),按照6个月随访临床结局分为死亡组(n=21)和存活组(n=39)。比较不同组别患者上述指标水平,采用Pearson相关性分析SDMA与AFP、CEA的相关性,绘制SDMA、AFP、CEA对肾囊腺瘤诊断的ROC曲线。结果:研究组Ⅰ、Ⅱ期和Ⅲ、Ⅳ期的患者SDMA、AFP和CEA水平均明显高于对照组,差异均有统计学意义(P<0.05);Ⅲ、Ⅳ期的SDMA、AFP和CEA水平均明显高于Ⅰ、Ⅱ期,差异均有统计学意义(P<0.05)。死亡组SDMA、AFP和CEA水平均明显高于存活组,差异均有统计学意义(P<0.05)。Pearson相关性分析结果显示,肾囊腺瘤患者血清SDMA和AFP、CEA均呈正相关(P<0.05)。ROC曲线结果显示,SDMA、AFP和CEA诊断肾囊腺瘤的AUC分别为0.8910、0.9221、0.9892。结论:血清SDMA水平对肾囊腺瘤具有较好的诊断价值,对患者预后有较好的预测价值,可考虑将其作为肾囊腺瘤的诊断与临床治疗参考指标进行推广应用。Objective:To explore the diagnostic and prognostic value of serum symmetrical dimethylarginine(SDMA)level in renal cystic adenoma.Method:A total of 60 patients with renal cystic adenoma treated in our hospital from January 2017 to January 2020 were selected as the study group,50 patients with benign renal lesions treated in our hospital in the same period were selected as the control group.The levels of SDMA,alpha fetoprotein(AFP)and carcinoembryonic antigen(CEA)in the two groups were detected.The patients in the study group were divided into stage Ⅰ,Ⅱ(n=37)and stage Ⅲ,Ⅳ(n=23)according to TNM stage;according to the clinical outcome of 6-month follow-up,they were divided into death group(n=21)and survival group(n=39).The above index levels of patients in different groups were compared,the correlation between SDMA and AFP,CEA was analyzed by Pearson correlation,the ROC curve of SDMA,AFP and CEA in the diagnosis of renal cystic adenoma was drawn.Result:The levels of SDMA,AFP and CEA in patients with stage Ⅰ,Ⅱ and stage Ⅲ,Ⅳ in study group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The levels of SDMA,AFP and CEA in patient with stage Ⅲ,Ⅳ were significantly higher than those in stages Ⅰ,Ⅱ,the differences were statistically significant(P<0.05).The levels of SDMA,AFP and CEA in the death group were significantly higher than those in the survival group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that there were positive correlations between serum SDMA and AFP,CEA in patients with renal cystic adenoma(P<0.05).The results of ROC curve showed that the AUC of SDMA,AFP and CEA in the diagnosis of renal cystic adenoma were 0.8910,0.9221 and 0.9892 respectively.Conclusion:The level of serum SDMA has good diagnostic value for renal cystadenoma and good predictive value for the prognosis of patients,it can be considered to be popularized and applied as a reference index for the diagnosis and
