儿童眼部肿瘤294例病理学分析

摘　　要：目的探讨儿童眼部肿瘤的常见种类、临床与病理分类特点。方法回顾性研究,对2015年1月至2019年12月湖南省儿童医院眼科收治的294例眼部肿瘤患儿的病理诊断根据肿瘤的发生部位、年龄及病理分型进行分类整理。结果294例患儿,平均年龄3.67岁,最小年龄1月龄,最大年龄15岁。1岁及以下的婴儿61例(20.75%),4岁及以下的幼儿161例(54.76%)。其中男性155例,女性139例。良性肿瘤285例(96.94%),恶性肿瘤9例(3.06%)。眼睑和眼表肿瘤138例(46.94%),其中常见的为毛母质瘤37例,角结膜皮样瘤21例,皮样囊肿或表皮样囊肿14例。而上皮乳头状瘤少见,仅1例。眼眶肿瘤147例(50.00%),良性肿瘤多见,良性肿瘤中最常见的为皮样囊肿141例。眼内肿瘤9例(3.06%),均为视网膜母细胞瘤。结论儿童眼部肿瘤以先天性和胚胎性肿瘤为主,良性肿瘤多见。儿童眼部肿瘤的高发人群为1至4岁婴幼儿,肿瘤常见发病部位为眼睑和眼表、眶周(皮样囊肿多见)。儿童恶性眼部肿瘤以视网膜母细胞瘤为主。Objective To analyze the pathological classification of ocular tumor in children.Methods 294 cases(294 eyes)of ocular tumor in children below 15 years old between January 2015 and December 2019 were reviewed retrospectively.The pathological classification were made according to the location,age and nature of the tumor.Results 294 children,which had 155 males and 139 females,were rolled in.The mean age was 3.67 years old.The minimus’was1 month and the maximum’s was 15 years old.There were 61 cases below 1 years old(20.75%),and there were 161 cases under the age of 4 years old(besides infants)(54.76%).Benign tumors were found in 285 cases(96.94%)and malignant tumors in 9 cases(3.06%).138 eyes(46.94%)with benign tumor of eyelid,conjunctive and cornea,the leading tumor were 37 eyes with pilomatricoma.In which 21 eyes with corneal and conjunctival dermoid tumors;14 eyes of epidermoid cyst and dermoid cyst were included.Epithelial papilloma was rare(only 1 case).There were 147 cases of orbital tumor(50.00%),most of which were benign tumors.Dermoid cyst was the most common one in 141 cases.There were 9 cases(3.06%)of intraocular tumors,all of which were retinoblastoma.Conclusion The main ocular tumors in children are congenital and embryonal ones,and the benign ones are more common.The high incidence of eye tumor in children is 1 to 4 years old infants,the common site of tumor is eyelid and ocular surface,orbit(dermoid cyst is more common).Retinoblastoma is the main malignant eye tumor in children.
