摘 要:目的研究骨皮质切开术在骨性Ⅱ类错[牙合]畸形患者正畸治疗中的应用价值。方法纳入我院2015年1月至2019年12月收治的40例骨性Ⅱ类错[牙合]畸形患者为研究对象,按照随机数字表法将其分为观察组和对照组,各20例。观察组采用骨皮质切开术辅助正畸治疗,对照组采用常规正畸治疗。比较两组上牙列排齐时间、上牙列间隙关闭时间、矫治总时间,矫正前后牙位置、硬组织及软组织改善情况,矫正前后上气道各段表面积、体积、高度及舌骨位置。结果观察组上牙列间隙关闭时间及矫治总时间均显著短于对照组(P<0.05)。观察组的U1I-CP、U1R-CP、Ls-HP、Ls-CP、Sd-CP、SNA、UARA改变量显著大于对照组(P<0.05)。矫正后,观察组的鼻咽部表面积显著大于对照组,口咽部体积小于对照组,舌骨垂直向距离大于对照组(P<0.05)。结论骨皮质切开术辅助治疗骨性Ⅱ类错[牙合]畸形患者,对提高矫正效果、缩短矫治时间具有重要临床价值。Objective To study the application value of cortical osteotomy in orthodontic treatment of patients with skeletal class Ⅱ malocclusion.Methods Forty patients with skeletal class Ⅱ malocclusion treated in our hospital from January 2015 to December 2019 were included as the research objects and divided into observation group and control group according to random mumber table,with 20 cases in each group.The observation group was treated with cortical osteotomy assisted orthodontic treatment,and control group was treated with conventional orthodontic treatment.The alignment time of upper dentition,closing time of upper dentition space and total correction time,the improvement of tooth position,hard tissue and soft tissue before and after correction,and the surface area,volume,height of each segment of upper airway and hyoid position before and after correction were compared between the two groups.Results The closing time of upper dentition space and total correction time in the observation group were significantly shorter than those in the control group(P<0.05).The changes of U1I-CP,U1R-CP,Ls-HP,Ls-CP,Sd-CP,SNA and UARA in the observation group were significantly greater than those in the control group(P<0.05).After correction,the surface area of nasopharynx in the observation group was greater than that in the control group,the volume of oropharynx was smaller than that in the control group,and the vertical distance of hyoid bone was significantly greater than that in the control group(P<0.05).Conclusion Cortical osteotomy as an auxiliary treatment for patients with skeletal class Ⅱ malocclusion has important clinical value in improving the correction effect and shortening the correction time.
