子宫背带缝合术联合子宫动脉上行支结扎术治疗难治性产后出血的临床效果

摘　　要：目的探讨子宫背带缝合术联合子宫动脉上行支结扎术治疗难治性产后出血的临床效果。方法选取2018年1月至2020年6月我院妇产科收治的120例难治性产后出血患者,采用随机数字表法将其分为对照组和观察组,各60例。对照组实施子宫动脉上行支结扎术,观察组在其基础上联合子宫背带缝合术治疗。比较两组的治疗效果。结果观察组的止血有效率高于对照组,并发症总发生率低于对照组(P<0.05)。观察组的术后恶露持续时间、下床活动时间、住院时间均短于对照组(P<0.05)。治疗后,观察组的血流阻力指数低于对照组,收缩期血流速度、舒张末期血流速度高于对照组(P<0.05)。治疗后,观察组的红细胞计数、血红蛋白水平、平均动脉压、心率、体温均高于对照组(P<0.05)。结论子宫背带缝合术联合子宫动脉上行支结扎术可提高难治性产后出血患者的止血效果,减轻卵巢血供损伤,减少并发症发生风险,促使患者血常规和生命体征恢复正常。Objective To investigate the clinical effect of uterine sling suture combined with ligation of ascending branch of uterine artery in the treatment of intractable postpartum hemorrhage.Methods A total of 120 patients with intractable postpartum hemorrhage admitted in the obstetrics and gynecology department of our hospital from January 2018 to June 2020 were selected and divided into control group and observation group by random number table method,with 60 cases in each group.The control group was treated with ligation of ascending branch of uterine artery,and the observation group was treated with uterine sling suture on the basis.The therapeutic effects of the two groups were compared.Results The effective rate of hemostasis in the observation group was higher than that in the control group,and the total incidence of complications was lower than that in the control group(P<0.05).The postoperative lochia duration,out of bed activity time and hospital stay in the observation group were shorter than those in the control group(P<0.05).After treatment,the blood flow resistance index in the observation group was lower than that in the control group,and the systolic blood flow velocity and end diastolic blood flow velocity in the observation group were higher than those in the control group(P<0.05).After treatment,the red blood cell count,hemoglobin level,mean arterial pressure,heart rate and body temperature in the observation group were higher than those in the control group(P<0.05).Conclusion Uterine sling suture combined with ligation of ascending branch of uterine artery can improve the hemostatic effect of patients with intractable postpartum hemorrhage,reduce the damage of ovarian blood supply,reduce the risk of complications,and promote the recovery of blood routine and vital signs of patients.
