脾硬化性血管瘤结节性转化的CT表现与病理对照分析

摘　　要：目的探讨脾硬化性血管瘤结节性转化(SANT)的CT表现和病理特点,提高该病的临床诊断水平。方法回顾性收集经手术病理确诊为脾SANT的患者8例,分析其CT表现并与病理结果进行对照分析。结果8例SANT的CT表现为脾内单发低密度影,边界光整,无囊变、坏死其中,3例呈类圆形或椭圆形,5例形态不规则,边缘可见分叶状,1例见点状钙化影,1例见“炸面圈”样钙化CT增强提示,3例病灶动脉期边缘明显强化,静脉期、延迟期向心性强化,病灶中央见无强化星芒状低密度影;5例病灶动脉期结节样、分隔状强化,条状分隔呈“轮辐状”持续强化,1例病灶动脉早期边缘出现血管影伸入病灶内部。结论SANT的CT表现具有一定特征性,仔细分析CT表现有助于提高诊断的准确性。Objective To investigate the CT manifestations and pathology features of sclerosing angiomatoid nodular transfomiation(SANT),so as to improve diagnostic accuracy of this disease.Methods 8 cases of splenic SANT diagnosed by surgical pathology were retrospectively collected,and their CT findings were analyzed and compared with pathological results.Results The characteristics of CT images were a single,non-cystic degeneration and necrosis hypodense lesion with well-defined margin.Among them,3 cases with round or oval shape,5 cases with irregular shape were lobulated.2 cases with calcification,one was localized punctuate,another was"doughnut-like.After enhancement,3 cases appeared peripheral enhancements with a central hypointenese stellate-like fibrous scar during arterial,venous and delayed phases.5 cases showed septate enhancements with a specific spoke-wheel appearance on enhancement imaging.A vessel stretched into the lesion center in 1 case.Conclusion Certain characteristics could be found on the CT manifestation on SANT,which could be helpful to improve the diagnostic accuracy of SANT by analyzing carefully.
