参苓白术散联合美沙拉嗪治疗溃疡性结肠炎临床观察

摘　　要：目的:观察参苓白术散联合美沙拉嗪治疗溃疡性结肠炎(ulcerative colitis,UC)的临床疗效及对相关血清指标的影响。方法:选择UC患者90例,随机分为对照组和观察组,每组45例。对照组给予美沙拉嗪治疗,观察组给予参苓白术散联合美沙拉嗪治疗,两组均连续治疗8周后评价并比较两组临床疗效及血清白细胞介素17(interleukin-17,IL-17)、白细胞介素23(interleukin-23,IL-23)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、血沉(erythrocyte sedimentation rate,ESR)、血小板(blood platelet,PLT)及D-二聚体水平。结果:总有效率观察组[88.89%(40/45)]优于对照组[71.11%(32/45)](P<0.05);治疗后,两组血清IL-17、IL-23、TNF-α、D-二聚体及ESR、PLT水平均明显低于治疗前(P<0.05),且观察组降低程度大于对照组(P<0.05)。结论:参苓白术散联合美沙拉嗪治疗UC临床疗效显著,且可降低血清炎症因子、ESR、PLT及D-二聚体水平。Objective:To observe clinical effects of Shenling Baizhu powder and mesalazine in the treatment of ulcerative colitis(UC)and its effects on the related serum indexes.Methods:All 90 UC patients were chosen and randomized into the control group and the observation group,45 cases in each group.The control group was treated by mesalazine,and the observation group was treated by Shenling Baizhu powder and mesalazine,both groups were treated for eight weeks consecutively.To assess and compare clinical effects,the levels of IL-17,IL-23,TNF-α,ESR,PLT and D-dimer between both groups.Results:Total effective rate of the observation group was 88.89%(40/45),superior to 71.11%(32/45)of the control group(P<0.05);after treating,the levels of IL-17,IL-23,TNF-α,D-dimer,ESR and PLT of both groups were lower than these before treating(P<0.05),and the decrease of the observation group was greater than that of the control group(P<0.05).Conclusion:Shenling Baizhu powder and mesalazine could gain notable effects in the treatment of UC,it could reduce the levels of serum inflammatory factor,the levels of ESR,PLT and D-dimer.
