超声造影增强模式鉴别局灶性胰腺炎与胰腺癌的临床价值:与常规超声的对照研究

摘　　要：目的:探讨超声造影增强和常规超声鉴别局灶性胰腺炎(Focal pancreatitis,FP)和胰腺癌(Pancreatic cancer,PC)的临床价值。方法:选择2015年1月—2020年1月在我院就诊的PC患者57例(PC组)和FP患者29例(FP组)作为研究对象,所有患者均给予常规超声和超声造影增强检查。对比分析增强超声造影对FP和PC的诊断灵敏度、特异性、准确率、阳性预测值(PPV)和阴性预测值(NPV)。结果:2组患者早期和晚期增强模式均存在统计学差异(P<0.05);FP患者早期增强模式以等强化和等强化伴局灶性增强为主,而PC患者以低强化和向心性强化为主;FP患者晚期以等强化和等强化伴局灶性增强为主,PC患者以低强化为主。等强化和等强化并局灶性增强相结合方式诊断FP的灵敏度、特异性、准确率、PPV和NPV均高于单纯等强化作为诊断标准。超声造影鉴别FP和PC的灵敏度、特异性、准确率、PPV和NPV均高于常规超声,差异均有统计学意义(P<0.05)。结论:超声造影增强模式能提高鉴别FP和PC的准确率,具有很高临床价值,值得临床推广应用。Objective:To investigate the clinical value of contrast-enhanced ultrasound and conventional ultrasound in the differential diagnosis of focal pancreatitis(FP)and pancreatic cancer(PC).Methods:Fifty-seven cases of PC patients(PC group)and 29 cases of FP patients(FP group)in our hospital from January 2015 to January 2020 were selected as the study objects.All patients were given conventional ultrasound and contrast-enhanced ultrasound examination.The sensitivity,specificity,accuracy,positive predictive value(PPV)and negative predictive value(NPV)of FP and PC were compared and analyzed.Results:There were significant differences in early and late enhancement modes between the two groups(P<0.05).The early enhancement mode of FP patients was mainly isoenhancement and isoenhancement with focal enhancement,while PC patients were mainly low enhancement and centripetal enhancement.In the late stage,the patients with FP were mainly isoenhancement and isoenhancement with focal enhancement,and PC patients were mainly low enhancement.The sensitivity,specificity,accuracy,PPV and NPV of the combination of isoenhancement and isoenhancement with focal enhancement were higher than those of isoenhancement alone.The sensitivity,specificity,accuracy,PPV and NPV of contrast-enhanced ultrasound were higher than those of conventional ultrasound(P<0.05).Conclusion:Enhancement mode of contrast-enhanced ultrasound improves the accuracy of differentiating FP from PC,which has high clinical value and is worthy of clinical application.
